摘　　要：目的探究以Roy适应模式为指导的管理干预对行血液透析治疗的重症狼疮性肾炎患者疾病管理能力、肾功能及免疫状态的影响。方法选取2019年6月至2022年6月期间我院肾内科收治的80例血液透析重症狼疮性肾炎患者作为研究对象,以抽签的方法分组。对照组40例给予常规干预,研究组40例给予以Roy适应模式为指导的综合管理。对比两组患者疾病管理能力、肾功能指标和免疫状态。结果研究组患者情绪管理、信息管理、日常生活管理、症状管理和自我效能分值均低于对照组(P<0.05);研究组患者血肌酐、红细胞沉降率和24 h尿蛋白定量水平均低于对照组(P<0.05);研究组患者免疫球蛋白A、免疫球蛋白G、免疫球蛋白M和双链DNA水平均高于对照组(P<0.05)。结论以Roy适应模式为指导的综合管理能够有效改善行血液透析治疗的重症狼疮性肾炎患者的肾功能及免疫状态,提高患者疾病管理能力。Objective To investigate the effects of Roy adaptation model on disease management ability,renal function and immune status in patients with severe lupus nephritis undergoing hemodialysis.Methods A total of 80 patients with severe lupus nephritis treated with hemodialysis from June 2019 to June 2022 were selected as the research subjects and grouped by lottery.Forty cases in the control group were given routine intervention,while 40 cases in the observation group were given combined comprehensive management guided by Roy adaptation model.The disease management ability,renal function indexes and immune status of the two groups were compared.Results The scores of emotion management,information management,daily life management,symptom management and self-efficacy in the observation group were lower than those in the control group(P<0.05).The levels of serum creatinine(Scr),erythrocyte sedimentation rate(ESR)and 24 h urinary protein in the observation group were lower than those in the control group(P<0.05).The levels of immunoglobulin A(IgA),immunoglobulin G(IgG),immunoglobulin M(IgM)and double-stranded DNA(ds-DNA)in the observation group were higher than those in the control group(P<0.05).Conclusions Comprehensive management guided by Roy adaptation model can effectively improve the renal function and immune status of patients with severe lupus nephritis undergoing hemodialysis,and improve the ability of disease management.
