冠心病2型糖尿病患者血浆sCD36水平与炎症指标的相关性研究

摘　　要：目的探讨冠心病2型糖尿病患者血浆可溶性CD36(sCD36)水平与炎症指标的关系。方法回顾性分析杭州市第三人民医院2018年1月至2021年5月收治的冠心病2型糖尿病35例(冠心病2型糖尿病组)、冠心病35例(冠心病组),选择同期体检正常者35例为对照组。冠心病2型糖尿病组根据随访期间是否发生不良心血管事件分为预后良好亚组23例和预后不良亚组12例。分析sCD36与超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)的关系。结果冠心病2型糖尿病组糖化血红蛋白(HbA1C)、sCD36水平均高于冠心病组、对照组(均P<0.05);预后不良亚组HbA1C、sCD36水平均高于预后良好亚组(均P<0.05)。冠心病2型糖尿病sCD36与hs-CRP、IL-6、TNF-α均呈正相关(r=0.918、0.782、0.793,均P<0.05)。多重线性回归分析显示,hs-CRP、IL-6、TNF-α均为冠心病2型糖尿病患者sCD36水平的影响因素(均P<0.05)。结论冠心病2型糖尿病患者血浆sCD36水平与hs-CRP、IL-6和TNF-α及预后相关。Objective To investigate the relationship between plasma soluble CD36(sCD36)expression and inflammatory markers in patients with coronary heart disease(CHD)and type 2 diabetes mellitus(T2DM).Methods Clinical data of 35 patients with CHD and T2DM,35 patients with CHD and 35 healthy people who underwent physical examination in Hangzhou Third People's Hospital from January 2018 to May 2021 were retrospectively analyzed.Patients with CHD and T2DM were divided into good prognosis(n=23)and poor prognosis(n=12)according to whether adverse cardiovascular events occurred during follow-up period.The relationships between plasma sCD36 and high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6)and tumor necrosis factorα(TNF-α)were analyzed.Results The levels of HbA1C and sCD36 in patients with CHD and T2DM were significantly higher than those in patients with CHD and healthy people(all P<0.05).HbA1C and sCD36 in the poor prognosis group were significantly higher than those in the good prognosis group(all P<0.05).sCD36 was positively correlated with hs-CRP,IL-6 and TNF-αin patients with CHD and T2DM(r=0.918,0.782,0.793,all P<0.05).Regression analysis showed that hs-CRP,IL-6 and TNF-αwere influencing factors of sCD36(all P<0.05).Conclusion sCD36 is related to hs-CRP,IL-6,TNF-αand prognosis of patients with CHD and T2DM.
