关节镜下冻结肩松解术对老年患者血清BGP、IGF-1表达的影响

摘　　要：目的探讨与分析关节镜下冻结肩松解术对老年患者血清骨钙素(Bone-γ-Carboxyglutamic Acid-Containing Protein,BGP)、胰岛素样生长因子-1(Insulin-Like Growth Factor-1,IGF-1)表达的影响。方法选择2020年6月—2022年4月在辽宁省健康产业集团抚矿总医院诊治的原发性冻结肩老年患者76例作为研究对象,根据随机数表法把患者分为关节镜组38例与对照组38例。对照组给予常规闭合推拿术治疗,关节镜组给予关节镜下冻结肩松解术治疗,检测患者血清骨钙素、胰岛素样生长因子-1表达情况。结果治疗后1个月,关节镜组优良率明显高于对照组(P<0.05)。关节镜组与对照组治疗后1个月的肩关节前屈与外展程度明显高于治疗前(P<0.05),关节镜组明显高于对照组(P<0.05)。关节镜组与对照组治疗后1个月的肩关节疼痛VAS评分明显低于治疗前(P<0.05),治疗后1个月关节镜组明显低于对照组(P<0.05)。关节镜组与对照组治疗后1个月的血清骨钙素、胰岛素样生长因子-1含量都显著高于治疗前(P<0.05),治疗后1个月关节镜组的血清骨钙素、胰岛素样生长因子-1含量明显高于对照组(P<0.05)。关节镜组治疗后6个月的关节肿胀、关节感染、关节积血等并发症发生率明显低于对照组(P<0.05)。结论关节镜下冻结肩松解术在老年患者的应用能提高肩关节前屈与外展程度,改善患者的总体治疗优良率,也可促进缓解疼痛,降低并发症发生率,提高患者血清骨钙素、胰岛素样生长因子-1的表达水平。Objective To investigate and analysis the effect of arthroscopic frozen shoulder release on serum Bone-γ-Carboxyglutamic Acid-Containing protein(BGP)and insulin-like growth factor-1(IGF-1)expression.Methods From June 2020 to April 2022,76 cases of elderly patients with primary frozen shoulder who were diagnosed and treated in our hospital were selected as the study subjects.According to the random number table,the patients were divided into the arthroscopic group(38 cases)and the control group(38 cases).The control group were treated with conventional closed massage,and the arthroscopic group were treated with arthroscopic frozen shoulder release.The expression of serum osteocalcin and insulin-like growth factor-1 were detected.Results At one month after treatment,the excellent and good rate of the arthroscopic group were significantly higher than that of the control group(P<0.05).The degree of shoulder joint flexion and abduction in the arthroscopic group and the control group at one month after treatment were significantly higher than that before treatment(P<0.05),and the arthroscopic group were also significantly improved compared with the control group(P<0.05).The VAS score of shoulder joint pain in the arthroscopic group and the control group at one month after treatment were significantly lower than that before treatment(P<0.05),and the score of shoulder joint pain in the arthroscopic group were also significantly lower than that in the control group one month after treatment(P<0.05).The levels of serum Bone-γ-Carboxyglutamic Acid-Containing protein and insulin-like growth factor-1 in the arthroscopic group and the control group at one month after treatment were significantly higher than those before treatment(P<0.05),and the levels of serum Bone-γ-Carboxyglutamic Acid-Containing protein and insulin-like growth factor-1 in the arthroscopic group at one month after treatment were significantly higher than those in the control group(P<0.05).The incidence rates of joint swelling,joint infection,joint hemato
