颅内蛛网膜囊肿手术治疗方法的探讨

摘　　要：目的探讨颅内蛛网膜囊肿(IAC)的手术治疗方法、疗效及其适应证。方法回顾性分析手术治疗IAC 32例患者临床资料,其中囊肿切除术16例,囊肿分流术7例,囊肿造瘘术9例,总结IAC的手术方法、疗效及其适应证。结果32例IAC均成功施行手术治疗,术后随访3个月~6年,囊肿切除术16例中,15例(93.75%)术后囊肿缩小或消失,1例(6.25%)术后囊肿无明显变化,16例(100%)症状缓解或消失;囊肿分流术7例中,5例(71.43%)术后囊肿缩小或消失,2例(28.57%)术后囊肿复发,5例(71.43%)症状缓解或消失;囊肿造瘘术9例中,7例(77.78%)术后囊肿缩小或消失,1例(11.11%)术后囊肿无明显变化,1例(11.11%)术后囊肿复发,8例(88.89%)症状缓解或消失。结论手术治疗有临床症状的IAC具有良好的疗效,囊肿切除术是首选的手术治疗方法。Objective To investigate the efficacy and indication of surgical treatment managements of intracranial arachnoid cyst(IAC).Methods A retrospective analysis of clinical data of 32 patients with IACs undergoing surgery was performed,including 16 cases of cyst resection,7 cases of cyst shunt and 9 cases of cyst fistulation,and the efficacy and indication of surgical treatment managements of IAC were summarized.Results All the 32 patients with IAC were operated successfully,the postoperative follow-up ranged from 3 months to 6 years.Among the 16 patients who underwent cyst resection,15 patients(93.75%)had the cyst reduced or disappeared,and 1 patient(6.25%)had no significant change,and 16 patients(100%)of the symptoms were resolved or disappeared;Among the 7 patients of cyst shunt surgery,5 patients(71.43%)had cyst shrinkage or disappearance,2 patients(28.57%)had cyst recurrence,and 5patients(71.43%)had symptom remission or disappearance;In 9 patients of the cyst fistulation,the cyst shrank or disappeared in 7 patients(77.78%),remained unchanged in 1 patient(11.11%),recurred in 1 patient(11.11%),and the symptoms were resolved or disappeared in 8 patients(88.89%).Conclusion Surgical treatment of IAC with clinical symptoms has good effects,and cyst resection is the preferred surgical treatment management.
