改良拔尿管法在腰椎植骨融合内固定术后患者中的应用效果

摘　　要：目的探讨改良拔尿管法在腰椎植骨融合内固定术后患者中的应用效果。方法选择2019年1月至2021年1月我院120例进行腰椎植骨融合内固定术后留置尿管的患者作为研究对象,采用随机数字表法将其分为对照组和改良组,每组60例。对照组采用传统拔尿管法,改良组采用改良拔尿管法。比较两组的排尿顺畅情况、尿管留置时间、术后首次排尿时间、术后首次排尿量、肉眼血尿率、尿隐血试验阳性率、疼痛程度及并发症发生情况。结果拔除尿管后,改良组的排尿顺畅总有效率明显高于对照组(P<0.05)。改良组的尿管留置时间、术后首次排尿时间短于对照组,术后首次排尿量多于对照组(P<0.05)。术后首次排尿时、拔除尿管24 h后,改良组的肉眼血尿率及尿隐血试验阳性率均明显低于对照组(P<0.05)。拔除尿管后,改良组的疼痛程度明显轻于对照组(P<0.05)。拔除尿管24 h后,改良组的并发症总发生率明显低于对照组(P<0.05)。结论改良拔尿管法在腰椎植骨融合内固定术术后患者中的应用效果显著,可改善患者排尿顺畅情况,缩短尿管留置时间、术后首次排尿时间,增多术后首次排尿量,减少痛苦,降低并发症总发生率,值得推广应用。Objective To explore the application effect of modified catheter extraction method in patients after lumbar bone graft fusion and internal fixation.Methods A total of 120 patients with indwelling catheter after lumbar bone graft fusion and internal fixation in our hospital from January 2019 to January 2021 were selected as the research objects,the patients were divided into control group and improved group by random number table method,with 60 cases in each group.The control group was treated with traditional catheter removal method,and the improved group was treated with modified catheter extraction method.The smooth urination,catheter indwelling time,postoperative first urination time,postoperative first urination volume,gross hematuria rate,positive rate of urine occult blood test,pain degree and complications were compared between the two groups.Results After catheter removal,the total effective rate of smooth urination in the improved group was significantly higher than that in the control group(P<0.05).The catheter indwelling time and postoperative first urination time in the improved group were shorter than those in the control group,and the first postoperative urination volume was more than that in the control group(P<0.05).At the time of first postoperative urination and 24 h after catheter removal,the gross hematuria rate and positive rate of urine occult blood test in the improved group were significantly lower than those in the control group(P<0.05).After catheter removal,the pain degree of the improved group was significantly lighter than that of the control group(P<0.05).After 24 h of catheter removal,the total incidence of complications in the improved group was significantly lower than that in the control group(P<0.05).Conclusion The modified catheter extraction method in patients after lumbar bone graft fusion and internal fixation has significant application effect,it can improve the smooth urination of patients,shorten the catheter indwelling time,postoperative first urination time,increase the
