摘  要:目的探讨围术期营养管理联合心理干预对心脏外科手术患者心肺功能、营养状态及负性情绪的影响。方法选取2020年9月至2021年1月我院收治的40例心脏外科手术患者作为对照组,2021年2月至6月收治的40例心脏外科手术患者作为干预组。对照组采用常规护理,干预组在对照组基础上给予围术期营养管理联合心理干预。比较两组的临床指标、并发症发生情况、心肺功能指标、营养状态指标以及负性情绪。结果干预组的住院时长短于对照组,下床时间早于对照组,并发症总发生率低于对照组(P<0.05)。干预后,干预组的左室射血分数(LVEF)、心输出量(CO)、用力肺活量(FVC)和第1秒用力呼气容积(FEV_(1))均明显高于对照组(P<0.05)。干预后,干预组的血红蛋白(Hb)、白蛋白(ALB)、总蛋白(TP)水平高于对照组(P<0.05)。干预后,干预组的焦虑自评量表(SAS)和抑郁自评量表(SDS)评分低于对照组(P<0.05)。结论围术期营养管理联合心理干预可促进心脏外科手术患者心肺功能恢复,改善营养状态及负性情绪。Objective To explore the effects of perioperative nutritional management combined with psychological intervention on cardiopulmonary function,nutritional status and negative emotions in patients undergoing cardiac surgery.Methods A total of 40 patients undergoing cardiac surgery admitted in our hospital from September 2020 to January 2021 were selected as the control group,and 40 patients undergoing cardiac surgery from February to June 2021 were selected as the intervention group.The control group was given routine nursing,and the intervention group was given perioperative nutrition management combined with psychological intervention on the basis of the control group.The clinical indexes,complications,cardiopulmonary function indexes,nutritional status indexes and negative emotions of the two groups were compared.Results The hospitalization time of the intervention group was shorter than that of the control group,the time of getting out of bed was earlier than that of the control group,and the total incidence of complications was lower than that of the control group(P<0.05).After intervention,the left ventricular ejection fraction(LVEF),cardiac output(CO),forced vital capacity(FVC)and forced expiratory volume in one second(FEV_(1))in the intervention group were significantly higher than those in the control group(P<0.05).After intervention,the levels of hemoglobin(Hb),albumin(ALB)and total protein(TP)in the intervention group were higher than those in the control group(P<0.05).After intervention,the scores of Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)in the intervention group were lower than those in the control group(P<0.05).Conclusion Perioperative nutritional management combined with psychological intervention can promote the recovery of cardiopulmonary function in patients undergoing cardiac surgery,improve nutritional status and negative emotions.
