摘  要:目的探讨基质Gla蛋白(MGP)、肿瘤坏死因子-α(TNF-α)和白介素IL-1β(IL-1β)与维持性血透(MHD)患者心脏瓣膜钙化(CVC)的关系。方法选择2018年1月—2019年12月于本院血液净化中心规律维持性血液透析且并发主动脉瓣或二尖瓣钙化的50例患者设为钙化组,选取同期在本透析中心规律透析且无心脏瓣膜钙化的50例患者,设为非钙化组。检测心脏瓣膜钙化分度、MGP、TNF-a和IL-1β等指标。结果钙化组ALB和MGP显著低于非钙化组(P<0.05);而钙化组hs-CRP、TNF-α和IL-1β均显著高于非钙化组(P=0.000);Ⅲ度CVC组的ALB和MGP低于Ⅰ度钙化组(P<0.05),Ⅲ度CVC组的hs-CRP、TNF-α及IL-1β均高于Ⅰ度钙化组(P<0.05);二元Logistic回归分析显示hs-CRP、TNF-α及IL-1β越高,发生心脏瓣膜钙化的可能性越大;ALB和MGP越低,发生心脏瓣膜钙化的可能性越小。结论ALB、MGP、hs-CRP、TNF-α及IL-1β和MHD患者心脏瓣膜钙化密切相关,ALB和MGP是心脏瓣膜钙化的保护性因素。以上指标对MHD患者心脏瓣膜钙化的预测和防治有一定价值。Objective To investigate the relationship between matrix Gla protein(MGP),tumor necrosis factor-α(TNF-α)and interleukin-1β(IL-1β)and heart valve calcification(CVC)in patients with maintenance hemodialysis(MHD).Methods Fifty patients with aortic valve or mitral valve calcification who underwent regular hemodialysis in the blood purification center of the third people's hospital of Yunnan province from January 2018 to December 2019 were selected as the calcification group,and 50 patients without cardiac valve calcification who underwent regular dialysis in our dialysis center during the same period were selected as the non-calcification group.Cardiac valve calcification index,MGP,TNF-αand IL-1βwere detected.Results ALB and MGP in calcification group were significantly lower than those in non-calcified group(P<0.05).However,hs-CRP,TNF-αand IL-1βin calcified group were significantly higher than those in non-calcified group(P=0.000).The ALB and MBP in grade III CVC group were lower than those in grade I calcification group(P<0.05),and the hs-CRP,TNF-αand IL-1βin grade III CVC group were higher than those in grade I calcification group(P<0.05).Binary logistic regression analysis showed that the higher hs-CRP,TNF-αand IL-1β,the higher the possibility of calcification of heart valve was.The lower the ALB and MGP,the less likely heart valve calcification occur.Conclusions ALB,MGP,hs-CRP,TNF-αand IL-1βare closely related to heart valve calcification in elderly MHD patients.ALB and MGP are protective factors for heart valve calcification.The above indexes have certain value for the prediction and prevention of heart valve calcification in elderly MHD patients.
