摘　　要：[目的]总结分析蒋旭宏教授治疗桥本甲状腺炎(Hashimoto thyroiditis,HT)的中医辨治思路。[方法]通过查阅HT的相关资料、跟师学习、整理医案、复习相关中医文献,整理总结蒋师临床治疗HT的中医辨治经验,分析HT的中医认识、中医治疗原则和用药方法,并列举医案分析。[结果]蒋师结合HT的中医病因病机、发病特点、临床症状和分期,提出痰浊结聚为HT的基本病机,治疗中应以化痰祛浊、软坚散结为主要原则,并应贯穿疾病始终,强调辨病与辨证相结合,分期论治。所举验案,蒋师辨为痰浊结聚、阴阳不和之证,治拟化痰散结、调和阴阳之法,临床效果甚佳。[结论]蒋师基于中医病因病机,对HT进行辨证论治,遣方用药从痰浊出发,随证加减,充分体现了中医药治疗HT的优势,其经验具有推广价值。[Objective]To study and summarize Professor JIANG Xuhong's thoughts on traditional Chinese medicine(TCM)syndrome differentiation and treatment of Hashimoto thyroiditis(HT).[Methods]By consulting HT related data,learning from teachers,sorting out medical records and reviewing relevant TCM literature,this paper summarized Professor JIANG's experience of treating HT based on syndrome differentiation of TCM,and analyzed the TCM understanding,TCM treatment principles and medication methods of HT with a medical case.[Results]According to TCM etiology and pathogenesis characteristics,clinical symptoms and installment,Professor JIANG proposes that phlegm-turbidity accumulation is the basic pathogenesis.Resolving phlegm and turbidity,resolving hard mass should be a main principle of treatment throughout the disease.The combination of disease differentiation and syndrome differentiation should be emphasized.In the attached medical case,Professor JIANG identified the syndrome as phlegm-turbidity accumulation,Yin and Yang disharmony,and the treatment mainly focused on resolving phlegm and stagnation,harmonizing Yin and Yang,and the curative effect was quite good.[Conclusion]Based on the etiology and pathogenesis of TCM,Professor JIANG treats HT based on syndrome differentiation,with the prescriptions based on phlegm-turbidity accumulation,modified with the syndrome according to the change of the condition,which fully reflects the advantages of TCM in the treatment of HT,and his experience has popularization value.
