摘　　要：目的探讨C-反应蛋白(CRP)是否与甲型流感或乙型流感病情相关,并评价CRP检测诊断ILI的价值。方法选择2020年11月至2021年1月季节性流感流行期间医院收治的ILI患儿进行分析,统计患者一般资料,分析CRP水平与症状的关联,探讨甲型或乙型流感与CRP水平的相关性。结果277例诊断流感样疾病患者中甲型流感121例(121/277,44%),乙型流感58例(58/277,21%)。根据CRP浓度无法确诊甲型流感患者,而CRP每增加10 mg/L则相应乙型流感显著降低,优势比为0.42(0.25~0.70,P<0.01)。临床症状上中重度症状呼吸急促、出汗、头晕等与CRP水平较高有关。结论CRP与甲型流感无关联关系,而CRP浓度增高与乙型流感风险降低有关。除非怀疑细菌感染时候,一般情况下不考虑利用CRP诊断ILI。Objective To investigate whether C-reactive protein(CRP)is correlated with influenza A or influenza B,and to evaluate the diagnostic potential of CRP in influenza-like illness(ILI).Methods Children with ILI admitted during the seasonal influenza epidemic from November 2020 to January 2021 were selected for analysis.The general data of patients were collected.The correlation between CRP level and symptoms,and the correlation between influenza A or B and CRP level were explored.Results The prevalence of positive tests for influenza A and B was 44%(121/277)and 21%(58/277),respectively.Patients with influenza A infection could not be identified based on CRP concentration.However,the incidence of influenza B decreased significantly with a 10 mg/L increase in CRP(adjusted odds ratio,0.42[0.25-0.70],P<.001).Moderate-to severe symptoms of ILI like shortness of breath,sweats and dizziness were associated with a higher CRP.Conclusion There is no association between CRP and influenza A.Increased concentration of CRP is associated with a lower risk for having influenza B.It is not recommended to diagnose ILI by measuring CRP,unless bacterial pneumonia is suspected.
