摘  要:目的了解河北省石家庄地区0~16岁儿童8中常见呼吸道病原体特异性IgM的检测情况,指导临床诊治。方法收集2019年9月至2020年8月因呼吸道感染就诊的0~16岁患儿2835例血清标本,采用胶体金法检测肺炎支原体(MP)、肺炎衣原体(CP)、甲型流感病毒(FluA)、乙型流感病毒(FluB)、副流感病毒(HPIV)、呼吸道合胞病毒(RSV)、腺病毒(HAdV)、柯萨奇病毒(COX)8种呼吸道病原体IgM抗体,比较不同季节、年龄段、性别感染率差异。结果2835里样本中共检出抗体阳性1968例(69.4%),其中单一病原体感染率38.8%(1101例),混合感染30.6%(867例)。所收集标本中,MP阳性率最高(31.4%),其次为FluA、FluB、HAdV、RSV、CP、COX和HPIV,分别为19.5%、17.7%、12.9%、10.1%、7.6%、7.3%和2.5%。不同年龄段儿童,除HPIV和COX之外,其他病原体感染率均有显著差异,其中学龄期组MP阳性率最高,婴儿组CP阳性率最高,FluA、FluB和HAdV幼儿组阳性率最高。该地区不同季节病原体感染率差异极显著(P<0.001),秋冬高于春夏。男童总体阳性率为53.7%(1057/1591),显著高于女童46.3%(911/1244)。结论掌握该地区0~16岁儿童呼吸道病原体感染的流行特点,有助于该类疾病的及时诊治及合理用药。Objective To investigate the detection on specific immunoglobulin M(IgM)antibody of eight species of respiratory tract pathogens in children(0-16 years)to guide clinical diagnosis and treatment in Shijiazhuang,Hebei.Methods The serum specimens of 2,835 children aged 0-16 years with respiratory tract infection were enrolled.The colloidal gold method was performed to detect IgM antibody of respiratory tract pathogens including mycoplasma pneumoniae(MP),chlamydia pneumoniae(CP),influenza A virus(FluA),influenza B virus(FluB),human parainfluenza virus(HPIV),respiratory syncytial virus(RSV),human adenovirus(HadV),and coxsackie virus(COX).The infection rate of different seasons,age and gender was compared.Results There were 1,968/2,835(69.4%)of antibody positive infection,1,101/2,835(38.8%)of single pathogen infection,and 867/2,835(30.6%)of mixed infection among the samples.MP had the highest positive rate followed by FluA,FluB,HAdV,RSV,CP,COX and HPIV(31.4%vs 19.5%vs 17.7%vs 12.9%vs 10.1%vs 7.6%vs 7.3%vs 2.5%,respectively).Apart from HPIV and COX,the difference was statistically significant in infection rate of other pathogens in children of different ages.The school-age group and infant group had the highest positive rate of MP and CP,respectively.The early childhood group had the highest positive rate of FluA,FluB and HAdV.The pathogens infection rate in autumn and winter in Shijiazhuang was significantly higher than that in spring and summer(P<0.001).The overall positive rate in the male children was significantly higher than that in the female children(53.7%[1,057/1,591]vs 46.3%[911/1,244]).Conclusion The study on the epidemic characteristics of respiratory pathogen infection in children aged 0-16 years in Shijiazhuang is helpful for timely diagnosis and treatment and rational therapy.
