摘　　要：目的探讨中医辨证分型治疗女童性早熟的临床效果。方法将100例性早熟女童按中医辨证分型分为阴虚火旺型组50例、肝郁化火型组30例和痰湿壅滞型组20例,分别给予医院自制早熟1号、2号和3号中药制剂治疗,疗程3个月。比较3组患儿的临床疗效及治疗前后乳核直径、子宫和卵巢容积、骨龄、中医证候积分。结果100例患儿总有效率为73%,3组不同分型患儿总有效率比较差异无统计学意义(P>0.05)。治疗后3组患儿的乳核直径、子宫和卵巢容积均较治疗前减小(P<0.05);阴虚火旺型组患儿的骨龄、中医证候积分较治疗前降低(P<0.05),肝郁化火型和痰湿壅滞型组患儿治疗前后骨龄、中医证候积分比较差异无统计学意义(P>0.05)。3组治疗期间随访均未见明显不良反应。结论中医辨证分型治疗女童性早熟临床疗效肯定,可以延缓性早熟女童第二性征及骨龄发育。Objective To investigate the clinical effect of TCM syndrome differentiation on precocious puberty in girls.Methods According to the TCM syndrome differentiation,100 girls with precocious puberty were divided into Yin-deficiency fire-excess type(50 cases),liver depression forming fire type(30 cases),and phlegm-dampness stagnation type(20 cases).The three groups were treated with preparations No.1,No.2 and No.3 for 3 months,respectively.Clinical efficacy was compared among the three groups.In addition,mammary nodule diameter,uterine volume,ovarian volume,bone age and TCM symptom score were determined before and after treatment.Results The total effective rate was 73%among the 100 girls.There was no significant difference in the total effective rate among the three groups(P>0.05).The diameter of mammary nodule and the volume of uterine and ovarian were reduced after treatment(P<0.05).Bone age and TCM symptom score were decreased after treatment in Yin-deficiency fire-excess type group(P<0.05),but not changed in liver depression forming fire type group and phlegm-dampness stagnation type group(P>0.05).No obvious adverse effects were found during the follow-up in all the three groups.Conclusion TCM syndrome differentiation treatment is effective for precocious puberty,and can delay the development of secondary sex characteristics and bone age in girls.
