中枢性性早熟女童中IGF-1和AMH的水平及意义

摘　　要：目的 研究中枢性性早熟女童中性激素、胰岛素样生长因子1(IGF-1)、抗缪勒氏管激素(AMH)水平的变化及各指标的诊断价值。方法 收集本院2022年1月至2022年10月门诊及住院的符合中枢性性早熟诊断的女童60例,年龄段在7.2±1.6岁,另收集健康女童45例,年龄段在7.1±1.5岁。受试者于空腹清晨采血,采用化学发光法检测性激素、IGF-1和AMH水平;对比组间水平,并采用ROC曲线计算LH、FSH、IGF-1、AMH诊断价值。结果 中枢性性早熟组中IGF-1和AMH水平较正常对照组升高,其中中枢性性早熟组中IGF-1水平为273.53±40.67ng/mL,AMH水平为2.84±0.86ng/mL;正常对照组IGF-1水平为229.89±39.24ng/mL,AMH水平为1.99±0.51ng/mL,差异均有统计学意义(P均<0.05)。中枢性性早熟女童组中LH诊断界值是0.42IU/L,灵敏度为78.72%,特异性为85.11%,AUC为0.852(0.764~0.917)。FSH诊断界值是2.05IU/L,灵敏度为78.72%,特异性为89.36%,AUC为0.834(0.743~0.903)。IGF-1诊断界值是228ng/mL,灵敏度为89.36%,特异性为57.45%,AUC为0.790(0.694~0.868)。AMH诊断界值是2.65ng/mL,灵敏度为55.32%,特异性为68.09%,AUC为0.610(0.504~0.709)。当4个指标联合检测时,AUC可达0.922。结论 中枢性性早熟女童血清IGF-1、AMH水平较正常女童升高;血清IGF-1、AMH、LH及FSH 4个指标检测对中枢性性早熟女童有诊断意义,联合检测还能提高诊断价值。Objective To study the changes of levels of sex hormones,insulin-like growth factor-1(IGF-1)and anti-Miao Lei’s hormone(AMH)in girls with central precocious puberty and their diagnostic values.Methods From January,2022 to October,2022,we collected 60 outpatients and inpatients with central precocious puberty,aged 7.2±1.6,as well as 50 healthy girls,aged 7.1±1.5.Blood samples were collected all subjects in the morning with an empty stomach,and levels of sex hormones,IGF-1 and AMH were detected by chemiluminescence method.Levels between groups were then compared.The diagnostic values of FSH,LH,IGF-1 and AMH were evaluated by ROC curve.Results Levels of IGF-1 and AMH in the central precocious puberty group were higher than those in the normal control group,and levels of IGF-1 and AMH in the central precocious puberty group were 273.53±40.67ng/mL and 2.84±0.86 ng/mL,respectively.In the normal control group,the level of IGF-1 was 229.89±39.24ng/mL,and the level of AMH was 1.99±0.51ng/mL,with a statistical significance(P<0.05).The cutoff value of LH diagnosis in girls with central precocious puberty was 0.42 IU/L,with the sensitivity 78.72%,the specificity 85.11%,and the AUC was 0.852(0.764-0.917).The diagnostic threshold of FSH was 2.05 IU/L,with the sensitivity 78.72%,the specificity 89.36%,and the AUC was 0.834(0.743-0.903).The diagnostic threshold of IGF-1 was 228 ng/mL,with the sensitivity 89.36%,the specificity 57.45%,and the AUC was 0.790(0.694-0.868).The diagnostic cut-off value of AMH was 2.65 ng/mL,with the sensitivity 55.32%,the specificity 68.09%,and the AUC was 0.610(0.504-0.709).When the four indexes were jointly detected,AUC reach 0.922.Conclusion Serum IGF-1 and AMH levels of girls with central precocious puberty are higher than those of normal girls.Serum IGF-1,AMH,LH and FSH are of diagnostic significance for girls with central precocious puberty,and combined detection can also improve the diagnostic value.
