摘 要:目的 基质辅助激光解吸电离飞行时间质谱仪(MALDI-TOF MS)是近年发展迅速的鉴定病原菌的新技术,具有快速、准确、高通量、方便、低成本的优越性,本研究评价MALDI Microflex鉴定腹泻病原菌的效果。方法 收集3家医院临床分离的腹泻病原菌379株,采用提取法对其进行鉴定,评价质谱的鉴定效果;选择其中200株病原菌进行直接涂抹法和提取法前处理,对比鉴定效果。结果 腹泻病原菌包括沙门菌属、气单胞菌属、志贺菌属、大肠埃希菌属、弧菌属、艰难梭菌属、邻单胞菌属、耶尔森菌属等8个菌属,28个菌种,MALDI-TOF MS技术鉴定腹泻病原菌整体在属水平鉴定正确率为87.07%,种水平鉴定正确率59.37%;不同菌属及不同种的鉴定正确率有差别,气单胞菌和沙门菌较好;提取法鉴定沙门菌和气单胞菌的正确率明显高于直接涂抹法(P<0.05)。结论 MALDI-TOF MS技术鉴定腹泻病原菌具有简单、快速、部分准确的优越性,需要扩充菌种库并结合生化及血清学鉴定。Objective Matrix-assisted laser desorption ionization time-of-flight mass spectrometry(MALDI-TOFMS)is a new technology that has developed rapidly in recent years to identify pathogenic bacteria.It has the advantages of fast,accurate,high-throughput,convenient and low-cost.This study evaluates the effect of MALDI Microflex in identifying pathogenic bacteria of diarrhea.Methods We collected 379 strains of diarrhoeal pathogenic bacteria isolated from 3 hospitals from clinical samples,and used the extraction method to identify them with Microflex,including 8 genera and 28 species of Salmonella,Aeromonas,Shigella,Escherichia coli,Vibrio,Clostridium difficile,Pseudomonas and Yersinia.The identification effect of mass spectrometry was evaluated.200 strains of pathogenic bacteria were then selected for pretreatment with direct smear method and extraction method,and the identification effect was compared.Results Pathogenic bacteria of diarrhea included 8 genera and 28 species,including Salmonella,Aeromonas,Shigella,Escherichia coli,Vibrio,Clostridium difficile,Pseudomonas and Yersinia.The overall accuracy rate of identification of pathogenic bacteria of diarrhea by MALDI-TOF MS technology was 87.07%at the genus level and 59.37%at the species level;The identification accuracy of different genera and species was different,Aeromonas and Salmonella showed a better performance.The correct rate of identification of Salmonella and Aeromonas by extraction method was significantly higher than that by direct smear method(P<0.05).Conclusion MALDI-TOF MS technology has advantages of simple,rapid and partially accurate identification of diarrhoeal pathogens.It is necessary to expand the strain bank and combine biochemical and serological identification.
