松解术加热效应对腰肌筋膜炎软组织张力特征影响的临床观察

摘　　要：目的探讨腰椎周围软组织张力测定在腰肌筋膜炎治疗过程中作为疗效评价重要客观指标的临床意义及松解术治疗腰椎筋膜炎的治疗机理。方法选取126例腰肌筋膜炎患者随机分为射频电极刃针组(观察组)和毫针针刺组(对照组)各63例,分别给予射频电极刃针松解术和毫针针刺干预。治疗前后采用VAS疼痛评分评价疗效,应用M-Tone软组织张力测试分析系统及MT-JZL-Ⅲ型肌张力测试传感器测试患侧压痛点在0.5 kg时所对应的压力位移及组织能量吸收比。结果治疗1周、6个月后,2组患者VAS评分、0.5 kg压力位移较治疗前明显降低(P<0.01),且观察组低于对照组(P<0.01,P<0.05);2组患者组织能量吸收比均较治疗前上升(P<0.01),且观察组高于对照组(P<0.05,P<0.01);治疗1周、6个月后观察组总有效率高于对照组(P<0.05)。结论射频电极刃针松解术可提高腰椎周围肌肉软组织刚度,增强其变形能力,从而恢复腰椎周围关节的力学平衡。Objective To evaluate the clinical significance of measurement of lumbar peripheral soft tissue tension in the treatment of lumbar fasciitis as an important objective index of curative effect evaluation and to investigate the therapeutic mechanism of lumbar fasciitis treated with lysis.Methods A total of 126 patients with lumbar fasciitis were randomly divided into a radiofrequency electrode blade needle group(observation group)and a filiform needle acupuncture group(control group),63 cases in each group.The observation group were treated with radiofrequency electrode blade needle lysis and the control group were treated with filiform needle acupuncture intervention.VAS pain score was used to evaluate the efficacy before and after treatment.M-Tone soft tissue tension test and analysis system and MT-JZL-Ⅲmuscle tension test sensor were used to test the pressure displacement and tissue energy absorption ratio corresponding to the tenderness point on the affected side at 0.5 kg.Results After 1 week and 6 months of treatment,the VAS score and 0.5 kg pressure displacement of patients in both groups significantly reduced compared to before treatment(P<0.01),and the data of the observation group were lower than those of the control group(P<0.01,P<0.05);the tissue energy absorption ratio of both groups increased compared to before treatment(P<0.01),and the data of the observation group was higher than that of the control group(P<0.05,P<0.01);after 1 week and 6 months of treatment,the total effective rate of the observation group was higher than that of the control group(P<0.05).Conclusion Radiofrequency electrode blade needle lysis increases the stiffness of the muscles and soft tissues around the lumbar spine,and enhances their deformation capacity,thus restoring the mechanical balance of the joints around the lumbar spine.
