摘  要:目的 观察益气活血通络方联合“足三里”针刺治疗对糖尿病神经病理性疼痛(diabetic neuropathic pain,DNP)大鼠背根神经节Nav1.7和Nav1.8表达的影响。方法 采用高脂高糖饲养联合单次小剂量腹腔注射链脲佐菌素复制2型糖尿病大鼠模型,通过检测大鼠的机械痛阈和热缩足反应时间判断DNP模型是否复制成功。将DNP大鼠随机分为模型组、针刺组、中药组、针药结合组。模型组予以生理盐水灌胃,针刺组针刺“足三里”20 min,中药组予以益气活血通络方(9 g/kg)灌胃,针药结合组予以针刺“足三里”联合益气活血通络方灌胃,另设空白组。每日治疗1次,每周治疗5 d,连续4周。治疗结束后取大鼠L_(4)—L_(6)背根神经节,Western blot法检测Nav1.7和Nav1.8蛋白表达水平,qRT-PCR法检测Nav1.7、Nav1.8 mRNA表达水平,免疫荧光染色法检测大鼠背根神经节中Nav1.7、Nav1.8的荧光表达水平。结果 与模型组比较,针刺组、中药组、针药结合组大鼠机械痛阈显著升高(P<0.05),热缩足反应时间显著延长(P<0.05),背根神经节中Nav1.7、Nav1.8表达水平显著降低(P<0.05);与针刺组、中药组比较,针药结合组大鼠机械痛阈显著升高(P<0.05),热缩足反应时间显著延长(P<0.05),背根神经节中Nav1.7、Nav1.8表达水平显著降低(P<0.05)。结论 益气活血通络方联合针刺治疗DNP的效果优于单一疗法,其作用机制与抑制背根神经节中Nav1.7、Nav1.8表达有关。Objective To investigate the effect of Yiqi Huoxue Tongluo prescription combined with acupuncture at Zusanli on Nav1.7 and Nav1.8 in the dorsal root ganglia of rats with diabetic neuropathic pain(DNP).Methods Rats were given high-fat high-glucose diet combined with a single intraperitoneal injection of small-dose streptozotocin to establish a model of type 2 diabetes,and mechanical pain threshold and thermal withdrawal latency were measured to determine whether the model of DNP was successfully established.DNP rats were randomly divided into model group,acupuncture group,traditional Chinese medicine(TCM)group,and acupuncture-medicine group.The rats in the model group were given normal saline by gavage,those in the acupuncture group were given acupuncture at Zusanli for 20 minutes,those in the TCM group were given Yiqi Huoxue Tongluo prescription(9 g/kg)by gavage,and those in the acupuncture-medicine group were given acupuncture at Zusanli combined with Yiqi Huoxue Tongluo prescription by gavage,once a day and 5 days a week for 4 consecutive weeks.A blank group was also established.After the treatment ended,the L 4-L 6 dorsal root ganglia of the rats were collected;Western blot was used to measure the protein expression levels of Nav1.7 and Nav1.8,qRT-PCR was used to measure the mRNA expression levels of Nav1.7 and Nav1.8,and immunofluorescent staining was used to measure the fluorescence expression levels of Nav 1.7 and Nav1.8 in the dorsal root ganglia of rats.Results Compared with the model group,the acupuncture group,the TCM group,and the acupuncture-medicine group had significant increases in mechanical pain threshold and thermal withdrawal latency(P<0.05)and significant reductions in the expression levels of Nav1.7 and Nav1.8 in the dorsal root ganglia(P<0.05);compared with the acupuncture group and the TCM group,the acupuncture-medicine group had significant increases in mechanical pain threshold and thermal withdrawal latency(P<0.05)and significant reductions in the expression levels of Nav1.7 and Nav1.8 i
