艾滋病合并马尔尼菲青霉菌感染患者外周血T细胞因子检测的应用分析

摘　　要：目的:检测艾滋病合并马尔尼菲青霉菌感染患者外周血细胞因子水平,分析细胞因子对该疾病的诊断效能。方法:选择广州医科大学附属市八医院单纯AIDS患者73例,合并马尔尼菲青霉菌患者40例,采用美国BD公司BDFACSCantoⅡ流式细胞仪检测外周血T细胞因子IL-2、IL-4、IL-6、IL-10、TNF-α和IFN-γ,商品化试剂检测G实验等其他相关指标,统计分析细胞因子表达水平、各个指标单独及联合对艾滋病合并马尔尼菲青霉菌感染的诊断效能。结果:艾滋病合并马尔尼菲青霉菌感染患者外周血中T细胞因子和G实验、PCT和CRP指标变化相似,均显著升高(P<0.05),而Lymphocyte和PLT均显著降低(P<0.05),IL-6、IL-10单独检测对艾滋病合并马尔尼菲青霉菌感染的诊断效能和G实验、PCT和CRP指标相似,细胞因子和G实验等指标的联合检测对该疾病的敏感度、特异度分别为0.895、0.723。结论:T细胞因子可作为艾滋病和并马尔尼菲青霉菌感染的辅助诊断指标,并且为提高对该疾病诊断的准确性,建议细胞因子和G实验等指标联合检测。Objective:To detect the cytokines level of peripheral blood in patients infected with AIDS and Marnifi penicillin,and analyzes the diagnostic efficacy of cytokines in the disease.Methods:73 patients with AIDS and 40 cases infected with AIDS and Marnifi penicillin were selected from the eighth hospital of Guangzhou city of Guangzhou Medical University,and cytokines(IL-2,IL-4,IL-6,IL-10,TNF-αand IFN-γ)of Th1/2 were detected using BD FACS Canto II flow cytometers of BD corporation of the united states.Commercialized reagents were applied to detect G experiments and other relevant indicators.Cytokines expression levels and individual and joint indicators of the diagnostic efficacy were analyzed.Results:Th1/2 cytokines and G-experiments,PCT and CRP indicators were all significantly increased in the peripheral blood of patients with AIDS and Manifi penicillin infection(P<0.05),while Lymphocyte and PLT decreased significantly(P<0.05).The diagnostic efficacy of IL-6,IL-10 and G experiments et al.are similar for AIDS and Malnifi penicillin infection.The joint detection sensitivity and specificity of the cytokines and G experiments et al.are 0.895 and 0.723.Conclusion:Th1/2 cytokines can be used as auxiliary diagnostic indicators for AIDS and Malnifi penicillin infection.Furthermore,to improve the accuracy of diagnosis of the disease,it is recommended that cytokines and G experiments are detected together.
