耳穴揿针对腹腔镜下胆囊切除术后恢复的影响

摘　　要：目的:观察耳穴揿针对腹腔镜下胆囊切除患者术后恢复和自主神经功能的影响。方法:以随机数字表法将58例腹腔镜下胆囊切除术后患者分为对照组和观察组各29例。对照组给予抗感染、补液等常规治疗,观察组在常规治疗基础上,加以耳穴揿针治疗。比较两组患者肠鸣音恢复时间、首次肛门排气时间、首次排便时间,及术后不同时间点视觉模拟评分法(VAS)评分、血清肿瘤坏死因子-α(TNF-α)水平、血清胰多肽(PP)水平和血清去甲肾上腺素(NE)水平。结果:与对照组相比,观察组的肠鸣音恢复时间、首次肛门排气时间、首次排便时间更早(P<0.05);治疗后,观察组各时间点的VAS评分、血清TNF-α和NE水平均低于对照组(P<0.05),血清PP水平高于对照组(P<0.05)。结论:对腹腔镜胆囊切除术后患者施以耳穴揿针疗法,能够促进胃肠功能恢复、缓解疼痛、减轻炎症,其作用机制可能与调节自主神经功能有关。Objective:To investigate the effect of auricular acupuncture of press-needle on postoperative recovery and autonomic nerve function in patients after laparoscopic cholecystectomy.Methods:Totally 58 patients after laparoscopic cholecystectomy were randomly divided into observation group and control group by random number table method,with 29 cases in each group.Patients in the control group were given conventional treatment including anti-infection and fluid infusion etc.,and patients in the observation group were given conventional treatment combined with auricular acupuncture of press-needle.The time of resuming peristaltic sound,the first anus exhausting and the first defecation,the post-operation different time points visual analogue scale(VAS)and the levels of serum tumor necrosis factor-α(TNF-α),pancreatic polypeptide(PP)and norepinephrine(NE)after surgery were assessed.Results:In comparison with the control group,the time of resuming peristaltic sound,the first anus exhausting and the first defecation of patients in observation group were earlier(P<0.05),and the VAS score,the levels of serum TNF-αand NE were lower(P<0.05),the level of serum PP was higher(P<0.05).Conclusion:Auricular acupuncture of press-needle after laparoscopic cholecystectomy can promote gastrointestinal function recovery,relieve pain and reduce inflammation,and its mechanism may be related to the regulation of autonomic nervous function.
