舌针联合会厌通络汤治疗缺血性脑卒中后吞咽障碍疗效观察

摘　　要：目的:观察舌针联合会厌通络汤治疗缺血性脑卒中(IS)患者吞咽障碍的临床疗效。方法:选择脑卒中后吞咽困难的患者77例,按随机数字表法分为对照组41例和观察组36例,对照组接受常规治疗,观察组在对照组基础上采用舌针联合会厌通络汤治疗,连续治疗4周。采用饮水试验、藤岛一郎吞咽疗效评价量表、标准吞咽功能评价量表(SSA)评估疗效。结果:治疗后,饮水试验结果显示,观察组吞咽功能优于对照组;治疗后两组SSA评分均较前下降,且观察组低于对照组(P<0.05);治疗后两组藤岛一郎吞咽疗效评分均较前上升,且观察组高于对照组(P<0.05);治疗后,观察组总有效率91.67%、显效率69.44%,对照组总有效率82.93%、显效率34.15%,观察组疗效优于对照组(P<0.05)。结论:舌针联合会厌通络汤能够改善IS患者吞咽功能,临床效果显著。Objective:To observe the clinical efficacy of tongue acupuncture combined with Huiyan Tongluo Decoction(会厌通络汤)in the treatment of dysphagia after ischemic stroke.Methods:Total 77 patients with dysphagia after ischemic stroke were divided into control group(41 cases)and observation group(36 cases)according to the random number method.The patients in control group were given conventional treatment for 4 weeks,and the patients in observation group were treated with tongue acupuncture combined with Huiyan Tongluo Decoction on this basis for 4 weeks.The clinical efficacy and the scores of the water drinking test,Fujishima Ichiro swallowing efficacy evaluation scale and the standard swallowing assessment(SSA)were compared between the two groups before and after treatment.Results:After treatment,the water drinking test showed that the swallowing function of the patients in observation group was better than that of the control group.After treatment,SSA scores in both groups were lower than before,and observation group was lower than control group(P<0.05).After treatment,the Fujishima Ichiro swallowing efficacy score was increased in both groups,and the observation group was higher than the control group(P<0.05).The total effective rate of the observation group was 91.67%,apparent efficiency was 69.44%,and the total effective rate of the control group was 82.93%,apparent efficiency was 34.15%.The curative effect of the observation group was better than that of the control group(P<0.05).Conclusion:Tongue needle combined with Huiyan Tongluo Decoction can improve the swallowing function of patients with dysphagia after ischemic stroke,and the clinical effect is remarkable.
