摘　　要：目的 探究超声造影(CEUS)定量参数在肾移植排斥中的临床价值。资料与方法 回顾性分析2021年4—12月于兰州大学第二医院行CEUS的23例肾移植术后患者,根据临床诊断分为肾功能稳定组(n=13)、急性排斥组(n=3)和慢性排斥组(n=7),评估CEUS定量参数到达时间、上升支斜率、峰值强度、达峰时间、平均渡越时间、曲线下面积、曲线下洗入面积及曲线下廓清面积在肾移植排斥诊断中的辅助价值。结果 肾功能稳定组、急性排斥组、慢性排斥组峰值强度比较,差异有统计学意义(F=3.639,P=0.045),3组其余CEUS定量参数比较,差异无统计学意义(P均>0.05)。组间两两比较,与肾功能稳定组相比,急性排斥组的峰值强度更高(P=0.049)。结论 CEUS定量参数,特别是峰值强度,在肾移植排斥中有一定临床价值。Purpose To explore the clinical value of contrast-enhanced ultrasound(CEUS)quantitative parameters in patients with renal allograft rejection.Materials and Methods The CEUS data of 23 patients after kidney transplantation who underwent CEUS in Lanzhou University Second Hospital from April to December 2021 were retrospectively analyzed.All patients were divided into stable renal function group(n=13),acute rejection group(n=3)and chronic rejection group(n=7)based on clinical diagnosis as the final diagnosis.The CEUS quantitative parameters such as arrive time,slope,peak intensity(PI),time to peak,mean transit time,area under the curve,area under the curve wash in,and area under the curve wash out in the diagnosis of renal allograft rejection was evaluated.Results There was exclusively significant difference in PI among stable renal function group,acute rejection group,and chronic rejection group(F=3.639,P=0.045),while there were no significant differences in the rest of CEUS quantitative parameters among the three groups(all P>0.05).Additionally,the PI of acute rejection group was significantly higher than that of stable renal function group via post-hoc tests(P=0.049).Conclusion CEUS quantitative parameters,especially PI,have certain clinical value in renal allograft rejection.
