摘  要:目的观察推拿㨰法干预对骨骼肌急性钝挫伤兔肿瘤坏死因子-α(TNF-α)及鞘氨醇激酶-1(SphK1)、1-磷酸鞘氨醇受体3(S1PR3)表达的影响,探讨㨰法治疗骨骼肌钝挫伤的作用机制。方法将15只新西兰兔随机分为空白组、模型组和㨰法组,每组5只。模型组和㨰法组采用自制重力锤打击装置制备兔股四头肌急性钝挫伤模型。造模成功后第7日开始干预,㨰法组用自制㨰法按摩器在股四头肌损伤处进行㨰法按摩,3 min/次,每日上下午各1次,连续3 d,空白组和模型组捆绑相同时间。HE、Masson染色观察股四头肌损伤部位病理变化,ELISA检测股四头肌组织TNF-α含量,Western blot检测股四头肌组织SphK1、S1PR3蛋白表达。结果模型组肌纤维变形断裂,肌细胞坏死严重,肌膜模糊、不完整,且有肌纤维溶解后形成的空泡结构,炎性细胞浸润较明显,可见少量核居中的新生肌纤维,有大量胶原纤维;与空白组比较,模型组股四头肌组织TNF-α含量明显增加(P<0.01),SphK1、S1PR3蛋白表达明显升高(P<0.01)。与模型组比较,㨰法组炎性细胞浸润明显减轻,可见大量细胞核居中且大小不等的新生肌纤维及多核肌管,胶原纤维明显减少;股四头肌组织TNF-α含量明显减少(P<0.05),SphK1、S1PR3蛋白表达明显降低(P<0.05)。结论㨰法干预能通过下调骨骼肌急性钝挫伤兔受损组织TNF-α含量及SphK1、S1PR3表达,降低组织炎症反应,减缓纤维化,进而促进骨骼肌损伤修复。Objective To observe the effects of rolling massage manipulation on the content of TNF-αand the expressions of SphK1 and S1PR3 in acute blunt contusion of skeletal muscle in rabbits;To reveal the underlying mechanism in improving blunt contusion of skeletal muscle.Methods A total of 15 New Zealand white rabbits were randomly divided into blank group,model group,and rolling manipulation group,with 5 rabbits in each group.The models of acute blunt injury of quadriceps femoris were made in rabbits in model group and rolling manipulation group by using self-made gravity hammer.The rolling manipulation group used a self-made method massager to massage the injured quadriceps femoris muscle,3 minutes per time,once in the morning and afternoon,for 3 days.The blank group and the model group were bound for the same time.The pathological changes in quadriceps femoris of the lesion were observed by HE and Masson staining,the content of TNF-αin quadriceps femoris muscle tissue was detected by ELISA,SphK1 and S1PR3 protein expressions in quadriceps femoris muscle were detected by Western blot.Results The muscle fibers were deformed and broken in the model group,the muscle cells were necrotic seriously,the sarcolemma was blurred and incomplete,and there were vacuoles formed after the dissolution of muscle fibers,the infiltration of inflammatory cells was obvious,a small number of new muscle fibers with nucleus in the middle were visible,and there were a large number of collagen fibers.Compared with the blank group,the content of TNF-αin quadriceps femoris muscle tissue of model group significantly increased(P<0.01),and the expressions of SphK1 and S1PR3 protein significantly increased(P<0.01).Compared with the model group,the infiltration of inflammatory cells in the rolling manipulation group was significantly reduced,a large number of new muscle fibers and multinucleated myotubes with nuclei in the middle and different sizes were visible,and the collagen fibers were significantly reduced;the content of TNF-αin quadriceps
