IgA肾病的中医“审因论治”策略

摘　　要：IgA肾病发病机制复杂,患者临床表现异质性大,基于症状表现的传统中医辨证论治疗效尚不满意。笔者基于中医理论和中医思维,结合IgA肾病发病机制的现代医学研究进展及中国中医科学院广安门医院临床经验,提出对该病病因病机的新认识,从审因为先、攘外安内、三焦定位和靶方靶药等方面梳理IgA肾病的中医辨治策略,以供临床参考。The pathogenesis of IgA nephropathy is complex,and the clinical manifestations of patients are heterogeneous.TCM treatment based on cause differentiation,which is based on symptoms and manifestations,has not achieved satisfactory efficacy.Based on TCM theory and TCM thought,combined with the progress of modern medical research on the pathogenesis of IgA nephropathy and clinical experience of Guang'anmen Hospital,China Academy of Chinese Medical Sciences,the authors proposed a new understanding of the etiology and pathogenesis of the disease.This article sorted out the TCM treatment strategies for IgA nephropathy from the aspects of“differentiating the causes first,fighting against the outside and settling the internal problems,positioning three energizer,and using target prescriptions and drugs”for clinical reference.
