中药组方补阳还五汤联合穴位针刺治疗气虚血瘀型中风

摘　　要：目的 分析中药组方联合穴位针刺治疗气虚血瘀型中风的临床疗效。方法 选择气虚血瘀型中风患者150例,随机分为对照组与观察组,各75例。对照组予常规治疗,观察组予常规治疗及中药组方补阳还五汤联合穴位针刺治疗,2组均治疗2周。比较2组治疗后疗效、肢体康复效果、气虚血瘀证症状评分、神经功能相关指标及炎症指标。结果 治疗2周后,观察组总有效率(92.00%,69/75)高于对照组(80.00%,60/75)(P <0.05)。治疗2周后,2组FuglMeyer运动量表(FMA)上肢、下肢评分、功能独立性评定量表(FIM)得分、血清γ-氨基丁酸(GABA)、脑源性神经营养因子(BDNF)水平均升高,且观察组更高(P <0.05)。治疗2周后,2组气虚血瘀证症状评分,血清缺氧诱导因子1α(HIF-1α)、神经元特异性烯醇化酶(NSE)、超敏C反应蛋白(hs-CRP)、基质金属蛋白酶9(MMP-9)、白细胞介素-6(IL-6)水平均降低,且观察组更低(P <0.05)。结论 气虚血瘀型中风患者经中药组方联合穴位针刺治疗可促进肢体康复,改善气虚血瘀证症状评分,可改善机体神经功能、抑制炎症反应,疗效确切。Objective To analyze the curative effect of traditional Chinese medicine formula combined with acupoint acupuncture in treating stroke of qi deficiency and blood stasis type.Methods 150 Patients with stroke of qi deficiency and blood stasis type were selected and divided into the control group and the observation group randomly,with 75 cases in each group.The control group received routine treatment,and the observation group received routine treatment and the traditional Chinese medicine formula Buyang Huanwu decoction combined with acupoint acupuncture.Both groups were treated for 2 weeks.The curative effect,limb rehabilitation effects,symptom scores of qi deficiency and blood stasis syndrome,neurological function-related indexes and inflammatory indexes were compared between 2 groups after treatment.Results After 2 weeks of treatment,the total effective rate of the observation group(92.00%,69/75)was higher than that of the control group(80.00%,60/75)(P<0.05).After 2 weeks of treatment,Fugl-Meyer assessment scale(FMA)upper and lower limb scores,functional independence measure(FIM)scale,serum levels ofγ-aminobutyric acid(GABA)and brain derived neurotrophic factor(BDNF)were increased in both groups,and the observation groups was higher(P<0.05).After 2 weeks of treatment,the scores of qi deficiency and blood stasis syndrome,and serum levels of hypoxia-inducible factor-1α(HIF-1α),neuron-specific enolase(NSE),high-sensitivity C-reactive protein(hs CRP),matrix metalloproteinase-9(MMP-9)and interleukin-6(IL-6)in both groups were all decreased,and the observation group was lower(P<0.05).Conclusion The combination of traditional Chinese medicine and acupoint acupuncture treatment can promote limb rehabilitation and improve the symptom scores of qi deficiency and blood stasis syndrome in stroke patients of qi deficiency and blood stasis type.Its mechanism is to improve the body's neurological function and inhibit inflammatory reactions.The therapeutic effect is definite.
