绝经后子宫肌瘤患者住院服务利用情况分析

摘　　要：目的:选取北京市某医院2019-2021年绝经后子宫肌瘤患者的住院数据,对其费用水平、费用结构和服务利用情况进行分析。方法:通过SPSS 26.0统计软件,采用t检验和卡方检验对绝经组和围绝经组的相关指标进行分析。结果:子宫肌瘤患者的住院次均费用为15092.41元,医保报销比例65.27%,次均个人负担5265.79元。绝经组与围绝经组相比较,在手术麻醉费用、检查治疗费用、住院天数、手术个数、手术时间、年龄分布、患病年限、临床表现、手术方式、合并高血压疾病等方面的差异有统计学意义;在住院费用、药品费用、术后天数、术后出血量、手术方法、合并内科疾病、合并其他疾病方面的差异无统计学意义。结论:绝经后患子宫肌瘤的高峰年龄为57至58岁,且费用负担持续增长,提示患者应密切关注临床症状,加强健康体检,以提高老年女性的健康水平。Objective:This study selected the inpatient data of postmenopausal uterine leiomyoma patients in a certainhospital in Beijing from 2019 to 2021 to analyze their cost level,cost structure and service utilization.Methods:Through SPSS 26.0,t test and chi square test were used to analyze the menopausal group and perimenopausal group.Results:The average hospitalization expense of hysteromyoma patients was 15,092.41yuan,the proportion of medical insurance reimbursement was 65.27%,the average personal burden was 5,265.79 yuan.Comparison of menopausal group and perimenopausal group,the cost of anesthesia,the cost of examination and treatment,the number of days in hospital,the number of operations,the time of operations,the distribution of age,the number of years of illness,the clinical manifestation,the way of operations and the complication of hypertension,there was no significant difference.In hospitalization cost,drug cost,postoperative days,postoperative bleeding volume,operative methods,medical diseases and other diseases,,there was no significant difference.Conclusion:The peak age of postmenopausal uterine leiomyoma is 57 to 58 years old,and the cost burden continues to increase.It is suggested that patients pay close attention to clinical symptoms,actively strengthen physical examination,and improve the health level of elderly women.
