糖尿病视网膜病变患者血清Periostin、αvβ3、25(OH)D_(3)水平变化及相关性分析

摘　　要：目的基于血清骨膜蛋白(Periostin)、整合素αvβ3、25-羟基维生素D_(3)[25(OH)D_(3)]指标,探究糖尿病视网膜病变(DR)患者血清Periostin、αvβ3、25(OH)D_(3)水平变化及相关性。方法选取2020年5月至2022年5月于本院进行诊治的178例糖尿病(DM)患者作为研究对象,依据是否伴有DR进行分组,伴有DR为观察组,未伴有DR为对照组,观察组依据病变程度分为DR1组、DR2组、DR3组;观察两组患者临床生化指标及血清Periostin、αvβ3、25(OH)D_(3)水平;观察患者血清Periostin、αvβ3、25(OH)D_(3)水平之间的相关性;采用二元Logistic回归方程观察DR的影响因素。结果观察组SBP、FPG指标水平及Periostin、αvβ3水平明显高于对照组,25(OH)D_(3)水平显著低于对照组(P<0.05);患者25(OH)D_(3)与Periostin、αvβ3水平均呈负相关,Periostin与αvβ3水平呈正相关;且DR与Periostin、αvβ3水平变化呈正相关,与25(OH)D_(3)表达呈负相关;血清Periostin、αvβ3均为DR的危险因素,25(OH)D_(3)为DR的保护因素。结论血清Periostin、αvβ3、25(OH)D_(3)指标在DR患者中均有异常表达,且3者之间具有一定的相关性,同时3个指标与DR间均具有相关性,这可以帮助医师对DR患者进行早期诊断,从而有利于医护人员及时采取诊治措施,提高患者疗效,改善患者预后,为临床诊治DR提供一定依据。Objective Based on expressions of serum periostin(Periostin),integrinαvβ3,and 25-hydroxyvitamin D3(25(OH)D3)indexes,to explore the levels of serum periostin,αvβ3,and 25(OH)D3 in patients with diabetic retinopathy(DR)and their correlations.Methods A total of 178 patients with diabetes mellitus(DM)who were diagnosed and treated in our hospital from May,2020 to May,2022 were selected as the research subjects for the study.These patients were divided into groups according to whether they were accompanied by DR.The control group and the observation group were divided into DR1 group,DR2 group and DR3 group according to the degrees of lesions;The clinical biochemical indexes and serum levels of Periostin,αvβ3 and 25(OH)D_(3)in the two groups were observed;The correlation between serum Periostin,αvβ3 and 25(OH)D_(3)levels was observed;Multivariate logistic regression analysis of the influencing factors of DR was conducted.Results Levels of SBP,FPG,Periostin andαvβ3 in the observation group were significantly higher than those in the control group,while the level of 25(OH)D_(3)was significantly lower than that in the control group(P<0.05);25(OH)D_(3)was negatively correlated with periostin andαvβ3 levels,while periostin was positively correlated withαvβ3 levels;DR was positively correlated with changes in periostin andαvβ3 levels,while negatively correlated with changes in 25(OH)D_(3)levels;Serum periostin andαvβ3 were risk factors for DR,and 25(OH)D_(3)was a protective factor for DR.Conclusion Serum periostin,αvβ3,25(OH)D_(3)indexes are abnormally expressed in DR patients,and there is a certain correlation between these three indexes.Further,these three indexes are correlated with DR;Early diagnosis of DR patients is helpful for medical staff to take timely diagnosis and treatment measures,increase the efficacy of patients,effectively alleviate the prognosis of patients,and provide a reference basis for clinical diagnosis and treatment of DR.
