摘　　要：目的探讨2型糖尿病肾病(T2DN)患者可溶性内皮糖蛋白(sEng)、脂蛋白相关磷脂酶A2(Lp-PLA2)和胰岛素(INS)表达水平及意义。方法回顾性分析2018年10月至2022年1月在我院住院并治疗的2型糖尿病患者120例设为观察组,其中2型糖尿病肾病患者80例为T2DN组,其他糖尿病患者40例为非T2DN组,另选40例同期来我院健康体检者为对照组,以病理活检为“金标准”。比较各组的性别、年龄、尿白蛋白排泄率(UAER)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL)、糖化血红蛋白(HbA1c)、空腹血糖(FBG)等临床资料;检测各组sEng、Lp-PLA2和INS水平,比较各组sEng、Lp-PLA2和INS表达水平对T2DN的意义。结果T2DN组UAER、TC、HbA1c、FBG水平均高于非T2DN组和对照组,且非T2DN组UAER、TC、TG、HbA1c、FBG水平均高于对照组(P<0.05);观察组的sEng、Lp-PLA2和INS均显著高于对照组(P<0.05);T2DN组的sEng、Lp-PLA2和INS均显著高于非T2DN组(P<0.05);sEng、Lp-PLA2和INS检测均具有较高的灵敏度、特异性,差异均有统计学意义(P<0.05)。结论T2DN的sEng、Lp-PLA2和INS表达水平明显增高,且均高于单纯糖尿病患者,而sEng、Lp-PLA2和INS为T2DN的危险因素,且其联合检测具有较高灵敏度、特异性,有助于临床诊断。Objective To investigate expression levels and significance of soluble endothelial glycoprotein(sEng),lipoprotein-associated phospholipase A2(Lp-PLA2)and insulin(INS)in patients with type 2 diabetic nephropathy(T2DN).Methods A retrospective analysis was conducted.120 patients with Type 2 diabetes who were hospitalized and treated in our hospital from October,2018 to January,2022 were included in the observation group,while 40 patients with Type 2diabetic nephropathy as T2DN group and 80 patients with other diabetes as non-T2DN group.Another 40 healthy patients who visited our hospital during the same period were selected as the control group.The pathological biopsy was used as the"gold standard".Gender,age,urinary albumin excretion rate(UAER),total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL),glycosylated hemoglobin(HbA1c),fasting blood glucose(FBG)and other clinical data were compared among all groups.SEng,LP-PLA2 and INS levels were detected in each group,and the significance of sEng,LP-PLA2 and INS expression levels on T2DN were evaluated.Results Levels of UAER,TC,HbA1c and FBG in T2DN group were higher than those in non-T2DN group and control group,and levels of UAER,TC,TG,HbA1c and FBG in non-T2DN group were higher than those in control group(P<0.05).SEng,LP-PLA2 and INS in the observation group were significantly higher than those in the control group(P<0.05).SEng,LP-PLA2 and INS in T2DN group were significantly higher than those in the non-T2DN group(P<0.05).SEng,LP-PLA2 and INS had a high sensitivity and specificity,and the differences were statistically significant(P<0.05).Conclusion sEng,LP-PLA2 and INS expression levels in T2DN are significantly increased,which are higher than those in patients with diabetes mellitus alone.SEng,LP-PLA2 and INS can be used as risk factors in combined detection for T2DN,with a high sensitivity and specificity in clinical diagnosis.
