改良B-lynch缝合术治疗凶险型前置胎盘临床疗效观察及对母婴结局的影响研究

摘　　要：目的 评估改良B-lynch缝合术治疗凶险型前置胎盘的临床疗效,并对母婴结局的影响进行观察。方法 于2020年11月至2022年3月甄选出商丘市妇幼保健院收治的凶险型前置胎盘81例产妇作为样本对象。根据剖宫产术式的不同分为两组,分别为参照组40例与研究组41例;参照组予常规剖宫产手术治疗,研究组则予以改良B-lynch缝合术治疗;判别对比两组产妇临床效果、手术相关指标以及不良母婴结局情况。结果 研究组临床总有效率(95.12%)高于参照组(80.00%)(P<0.05);研究组手术出血量(275.56±85.65)mL与术后出血量(45.65±5.02)mL均低于参照组,手术时间(34.50±8.10)min与住院时间(7.85±2.08)min均短于参照组,4项对比所得提示差异有统计学意义(P<0.001);且该组不良母婴结局发生率(4.88%)低于参照组(20.00%),对比所得结果差异有统计学意义(P<0.05)。结论 针对凶险型前置胎盘运用改良B-lynch缝合术治疗效果优异,能够有效提高临床效果,优化手术指标,降低不良母婴结局发生率。Objective To evaluate the clinical efficacy of modified B-lynch suture in the treatment of dangerous placenta previa,and to observe the effect of maternal and infant outcomes.Methods From November 2020 to March 2022,81 pregnant women with dangerous placenta previa admitted to the Department of Obstetrics and Gynecology of Shangqiu Maternity and Child Care Hospital were selected as sample objects.According to the different caesarean section operation type,they were divided into the reference group 40 cases and the study group 41 cases respectively;The reference group was treated with conventional cesarean section,while the study group was treated with modified B-lynch suture.The clinical effect,operation related indexes and adverse maternal and infant outcomes were compared between the two groups.Results The total clinical effective rate of the study group(95.12%) was higher than that of the control group(80.00%)(P<0.05).The operative blood loss(275.56±85.65) mL and postoperative blood loss(45.65±5.02) mL in the study group were both lower than those in the reference group,and the operative time(34.50±8.10) min and hospital stay(7.85±2.08) min were both shorter than that of the reference group.The results showed that the difference was statistically significant(P<0.001).The incidence of adverse maternal and infant outcomes in this group(4.88%) was lower than that of the reference group(20.00%),and the difference was statistically significant(P<0.05).Conclusion The modified B-lynch suture has excellent effect on the treatment of dangerous placenta previa,which can effectively improve the clinical effect,optimize the surgical indicators,and reduce the incidence of adverse maternal and infant outcomes.
