CT影像对髂静脉压迫综合征并急性髂股静脉血栓患者肺栓塞发病的预测价值

摘　　要：目的 分析CT影像对髂静脉压迫综合征(IVCS)并急性髂股静脉血栓(IFDVT)患者肺栓塞发病的预测价值。方法 回顾性分析2020年1月—2022年1月许昌市中心医院收治的110例IVCS并IFDVT患者的各项资料,其中48例患者合并肺栓塞,设为肺栓塞组,其余62例患者未合并肺栓塞,设为对照组。单因素和logistic多因素分析IVCS并IFDVT患者肺栓塞发病的危险因素,并ROC曲线分析CT影像对其的预测价值。结果 左髂总静脉狭窄率、心脏病史、D-二聚体水平、下腔静脉漂浮血栓为IVCS并IFDVT患者肺栓塞发病的危险因素(P<0.05)。ROC曲线分析结果显示,左髂总静脉狭窄率、心脏病史、D-二聚体水平、下腔静脉漂浮血栓对IVCS并IFDVT患者肺栓塞发病均有较佳的预测价值。结论 左髂总静脉狭窄率、心脏病史、D-二聚体水平、下腔静脉漂浮血栓为IVCS并IFDVT患者肺栓塞发病的危险因素,提示CT影像指标对其肺栓塞情况有较佳预测价值。Objective To analyze the predictive value of CT images for pulmonary embolism in patients with iliac vein compression syndrome(IVCS) complicated with acute iliac femoral vein thrombosis.Methods The data of 110 patients with IVCS combined with IFDVT who were treated in our hospital from January 2020 to January 2022 were retrospectively analyzed.Among them,48 patients with pulmonary embolism were set as pulmonary embolism group,and the remaining 62 patients without pulmonary embolism were set as control group.Univariate and logistic multivariate analysis of the risk factors for pulmonary embolism in patients with IVCS combined with IFDVT,and ROC curve analysis of the predictive value of CT images.Results History of heart disease,stenosis rate of left common iliac vein,floating thrombosis of inferior vena cava and D-dimer level were risk factors for pulmonary embolism in patients with IVCS and IFDVT(P<0.05).ROC curve analysis showed that history of heart disease,stenosis rate of left common iliac vein,floating thrombosis of inferior vena cava and D-dimer level had better predictive value for pulmonary embolism in patients with IVCS combined with IFDVT.Conclusion History of heart disease,stenosis rate of left common iliac vein,floating thrombosis of inferior vena cava and D-dimer level are risk factors for pulmonary embolism in patients with IVCS combined with IFDVT,suggesting that CT imaging indicators have better predictive value for pulmonary embolism.
