持续性心房颤动患者的动态心电图Lorenz散点图分析

摘　　要：目的分析持续性心房颤动(PAF)患者的动态心电图Lorenz散点图的图像特征及诊断价值。方法选择2020年7月至2022年7月遂川县人民医院收治的141例房颤患者,性别不限,年龄50~85岁,100例PAF患者为PAF组,41例非PAF患者为对照组,对比分析两组心电Lorenz散点图特征及心电Lorenz散点图B线斜率对PAF的诊断效能。结果PAF组动态心电图Lorenz散点图特征以单纯扇形为主,或合并与坐标轴平行的线形图;对照组则以棒球拍落入扇形为主,或棒球拍与扇形图共存。PAF组B线斜率为(0.551±0.171),对照组为(0.607±0.078),差异有统计学意义(P<0.05)。ROC曲线分析显示,B线斜率具有较高的诊断效能(P<0.05),ROC曲线下面积为0.757,cut-off值为0.524,敏感性为48.51%,特异性为97.50%。结论单纯PAF患者24 h动态心电图Lorenz散点图以单纯扇形多见,PAF患者B线斜率较非PAF患者低,且散点图B线斜率对PAF有较高的诊断意义。Objective To analyze the image characteristics and diagnostic value of Lorenz scatter chart in dynamic electrocardiogram of patients with persistent atrial fibrillation(PAF).Methods A total of 141 patients with atrial fibrillation admitted to Suichuan County People's Hospital from July 2020 to July 2022 were enrolled in the study,both of sexes,50-85 years old.100 patients with PAF were included in the PAF group and 41 patients without PAF were included in the control group.The characteristics of ECG Lorenz scatterplots and the diagnostic efficacy of the slope of B line of ECG Lorenz scatterplots for PAF were analyzed in the two groups.Results In the PAF group,the Lorenz scatterplot of dynamic electrocardiogram was characterized by simple sector or combined with linear graph parallel to the coordinate axis.In the control group,the bat racket mainly fell into the fan shape,or the bat racket coexisted with the fan shape.The slope of B line was(0.551±0.171)in the PAF group and(0.607±0.078)in the control group(P<0.05).ROC curve analysis showed that the slope of B line had high diagnostic efficacy(P<0.05),the area under ROC curve,cut-off value,sensitivity and specificity were 0.757,0.524,48.51%and 97.50%respectively.Conclusion Lorenz scatterplot of 24 h dynamic electrocardiogram in patients with simple PAF is more common with simple fan-shape,and B line slope in patients with PAF is lower than that in patients without PAF,and the B line slope of the scatterplot has high diagnostic significance for PAF.
