银屑病合并代谢综合征的相关研究进展

摘　　要：银屑病是一种慢性炎症性皮肤病,常反复发作、顽固难治。银屑病是由T淋巴细胞介导的炎症性皮肤病。银屑病除了皮肤损害外,还可以同时发生其他多种系统性疾病。银屑病患者发生代谢综合征(MS)的概率较高。MS是一组基于超重/肥胖、脂代谢异常、高血压、胰岛素抵抗等多种表现的综合征。二者同为炎症性疾病,可能通过共同的炎症通路,发生相同的病理改变。目前已经提出的机制包括:共有的遗传因素、常见的炎症途径、脂肪因子的分泌、胰岛素抵抗、脂蛋白组成及功能、氧化应激、血管生成、血液高凝状态等。随着临床研究的深入,银屑病和MS两者之间发病机制的相关性将逐渐被阐明。银屑病患者应进行各种代谢性指标筛查,包括体重、血脂、血糖、血压等。治疗银屑病时要警惕MS的发生,对已存在MS的患者,加强代谢疾病的相关治疗,以提高银屑病治疗效果,减少银屑病复发率。本文对银屑病合并MS的相关研究进展进行综述,以期为今后的相关诊疗提供参考。Psoriasis is a chronic inflammatory skin disease that often recurs and is stubborn refractory.Psoriasis is an inflammatory skin disease mediated by T lymphocytes.In addition to skin damage,psoriasis can cause a variety of other systemic diseases simultaneously.Psoriasis patients have a higher probability of developing metabolic syndrome(MS).MS is a group of syndromes based on multiple manifestations such as overweight/fat,abnormal lipid metabolism,hypertension,insulin resistance and so on.Both are inflammatory diseases,and may undergo the same pathological changes through a common inflammatory pathway,at present,the mechanism that have already been proposed include common genetic factors,common inflammatory pathways,adipokine secretion,insulin resistance,lipoprotein composition and function,oxidative stress,angiogenesis,blood hypercoagulant state,etc.With the deepening of clinical research,the relevance of the pathogenesis between psoriasis and MS will be gradually elucidated.Psoriasis patients should ungergo screening for various metabolic indicators,including weight,blood lipid,blood sugar,blood pressure,etc.When treating psoriasis,it is necessary to be alert to the occurrence of MS.For patients with existing MS,the relevant treatment of metabolic diseases should be strengthened to improve the treatment effect of psoriasis and reduce the recurrence rate of psoriasis.This article reviews the related research progress of psoriasis combined with MS,in order to provide reference for future related diagnosis and treatment.
