内固定术与人工髋关节置换术治疗老年髋部骨折的临床疗效比较

摘　　要：目的:比较内固定术与人工髋关节置换术治疗老年髋部骨折的临床疗效。方法:选取2021年1月—2022年9月北京市回民医院收治的老年髋部骨折患者90例为观察对象,根据随机数字表法分为对照组与研究组,各45例。对照组给予内固定术治疗,研究组给予人工髋关节置换术治疗。比较两组患者手术相关指标、疼痛程度、髋关节功能、治疗效果。结果:研究组手术时间、住院时间短于对照组,术中出血量、术后引流量少于对照组,差异有统计学意义(P=0.001);术后7 d,两组患者视觉模拟评分法评分均降低,研究组低于对照组,差异有统计学意义(P=0.001);术后3个月、6个月,两组患者Harris评分均升高,研究组高于对照组,差异有统计学意义(P<0.05);研究组治疗优良率高于对照组,差异有统计学意义(P<0.05)。结论:人工髋关节置换术治疗老年髋部骨折能获得更好的临床效果,改善患者髋关节功能,降低疼痛程度,缩短手术治疗时间及住院时间,减少术中出血量、术后引流量。Objective:To compare the clinical efficacy of internal fixation and artificial hip arthroplasty in the treatment of hip fractures in the elderly.Methods:Ninety cases of elderly hip fracture patients admitted to Beijing Huimin Hospital from January 2021 to September 2022 were selected as observation subjects and divided into control group and study group according to the random number table method,forty-five cases each.The control group was treated with internal fixation and the study group was treated with artificial hip arthroplasty.The surgery-related indexes,pain level,hip function and treatment effect were compared between the two groups.Results:The operation time and hospitalization time of the study group were shorter than those of the control group,and the intraoperative bleeding and postoperative drainage were less than those of the control group,with statistically significant differences(P=0.001).At 7 d postoperatively,the visual analog scoring method scores decreased in both groups,and the study group was lower than the control group,and the difference was statistically significant(P=0.001).At 3 and 6 months after surgery,the Harris score increased in both groups,and the study group was higher than the control group,and the difference was statistically significant(P<0.05).The excellent treatment rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Artificial hip arthroplasty for hip fracture in the elderly can achieve better clinical results,improve the function of the hip joint,reduce the degree of pain,shorten the surgical treatment time and hospitalization time,and reduce intraoperative bleeding and postoperative drainage.
