河南省首例人感染H5N6禽流感流行病学调查分析

摘　　要：目的分析河南省首例人感染H5N6禽流感病例流行病学特征,为人感染禽流感防控工作提供科学依据。方法收集2022年3月河南省首例人感染H5N6禽流感病例临床诊治信息、流行病学资料,采集病例、密切接触者及接触环境的相关标本进行病原学检测,运用描述流行病学方法进行分析。结果河南省首例人感染H5N6禽流感病例咽拭子样本、肺泡灌洗液样本检测结果为甲型流感病毒H5N6亚型阳性;病例在某食品厂从事肉鸭分割工作,食品厂采集的6份鸭脖样本中4份和病例工作所用案板样本1份均检出甲型流感病毒H5N6亚型;病例于4月8日死亡;83名密切接触者进行了为期10天的隔离医学观察,未发现继发病例。结论河南省首例人感染H5N6禽流感病例为本地感染,感染来源可能为职业暴露于禽类或屠宰加工环境,非人传人病例。Objective To analyze the epidemiological characteristics of the first case of human infection with H5N6 avian influenza in Henan,and provide scientific evidence for the prevention and control of human infection with avian influenza.Methods Clinical and the epidemiological data of the first case of human infection with H5N6 avian influenza in March 2022 were collected,and the specimens of the case,close contacts and the related environment were tested for pathogen detection in the laboratory.The data gathered were analyzed by using the method of epidemiological description.Results The case was diagnosed as human infection with H5N6 subtype of avian influenza based on the positive detection results of pharyngeal swab specimen and alveolar lavage fluid.H5N6 subtype of avian influenza virus was detected in duck necks(4/6)and 1 sample of cutting tables from the meat duck slaughter factory where the case had worked;the case died on April 8;No secondary infection was found among 83 close contacts after medical observation for 10 days.Conclusion The first case of human infection with avian influenza A(H5N6)in Henan is a local infection,the source of which probably is from occupational exposure to poultry or the poultry slaughtering and processing environment,and it isn't case of human to human transmission.
