高频超声在睾丸结核诊断中的价值

摘　　要：目的探讨高频超声在睾丸结核诊断中的临床价值。方法回顾性分析2019-2021年于郑州大学第一附属医院确诊的93例睾丸结核患者的常规超声(5 MHz)及高频超声(18 MHz)检查结果,以病理诊断为金标准,比较两种检查方法对睾丸结核的诊断准确率及对睾丸结核分型检查的一致性,并比较两种检查方法下睾丸结核的血流灌注Adler分级差异。结果高频超声对睾丸结核的诊断准确率为86.02%(80/93),常规超声的诊断准确率为74.19%(69/93),差异有统计学意义(P<0.05);两种检查方法对诊断睾丸结核分型的一致性较差(Kappa=0.382,P<0.05);两种检查方法下睾丸结核血流分级构成差异比较无统计学意义(P>0.05),Adler分级占比最高的为I级。结论高频超声较常规超声诊断睾丸结核具有准确性高、分辨率高及分型更客观等优点,在临床工作中具有十分重要的应用价值。Objective To explore the clinical value of high-frequency ultrasound in the diagnosis of testicular tuberculosis.Methods The conventional ultrasonographic(5 MHz)and high-frequency(18 MHz)ultrasonographic results of 93 patients with testicular tuberculosis diagnosed in the First Affiliated Hospital of Zhengzhou University from 2019 to 2021 were retrospectively analyzed.Taking pathological diagnosis as gold standard,the diagnostic accuracy of the two methods was compared;the consistency of the two methods in the classification of testicular tuberculosis was compared.The Adler grade of blood perfusion in testicular tuberculosis was compared between the two methods.Results The diagnostic accuracy of high-frequency ultrasound for testicular tuberculosis was 86.02%(80/93),and that of conventional ultrasound was 74.19%(69/93).The difference was statistically significant(P<0.05).The consistency between the two methods in diagnosing testicular tuberculosis was poor(Kappa=0.382,P<0.40).There was no significant difference in the composition ratio of blood flow grading of testicular tuberculosis between conventional ultrasound and high-frequency ultrasound(P>0.05),and the highest proportion of Adler grading was level I.Conclusions Compared with conventional ultrasound,high-frequency ultrasound has the advantages of high accuracy,high resolution and more objective classification in the diagnosis of testicular tuberculosis,which has very important value in clinical work.
