芪参还五胶囊配合针灸对脑出血患者脑血管功能及神经功能的影响

摘　　要：目的:探讨芪参还五胶囊配合针灸对脑出血患者脑血管功能及神经功能的影响。方法:选取120例脑出血患者为研究对象,按照治疗方案不同分为对照组和观察组,每组各60例。对照组患者予以常规治疗;观察组患者予以常规治疗+芪参还五胶囊配合针灸治疗,疗程均为4周。比较两组患者临床疗效、认知功能[简易精神状态检查(MMSE)评分]、神经功能[美国国立卫生研究院卒中量表(NIHSS)评分]、吞咽功能[吞咽功能评估量表(GUSS)评分]、脑血管功能(血管外周阻力、脑血管流速、脑血管流量)、炎性因子[基质金属蛋白酶9(MMP-9)、肿瘤坏死因子α(TNF-α)]水平、血肿周围低密度区体积及血肿体积。结果:观察组患者总有效率高于对照组(95.00%vs.76.67%,P<0.05)。治疗后,观察组患者NIHSS评分、血管外周阻力、MMP-9及TNF-α水平均低于对照组(P<0.05);血肿周围低密度区体积、血肿体积小于对照组(P<0.05);MMSE评分、GUSS评分高于对照组(P<0.05);脑血管流速快于对照组(P<0.05);脑血管流量大于对照组(P<0.05)。结论:芪参还五胶囊配合针灸可有效减轻患者神经功能损害,提高患者认知功能,改善患者吞咽功能,缓解患者脑血管功能,降低炎症因子水平,减少血肿引发的组织伤害,临床疗效较好。Objective:To investigate the effect of Qishenhuanwu capsule combined with acupuncture on cerebral vascular function and nerve function in patients with cerebral hemorrhage.Methods:According to different treatment plans,120 patients with cerebral hemorrhage were divided into control group and observation group,60 cases in each group.The control group was given routine treatment,and the observation group was conventional treatment+Qishenhuanwu capsule combined with acupuncture treatment.The clinical efficacy,cognitive function[Mini Mental State Examination(MMSE)score],neurological function[National Institutes of Health Stroke Scale(NIHSS)score],swallowing function[Swallowing Function Assessment Scale(GUSS)score],cerebrovascular function(peripheral vascular resistance,cerebral blood flow velocity,and cerebral blood flow),inflammatory factors[Matrix metalloproteinase-9(MMP-9),tumor necrosis factor-α(TNF-α)],the volume of low-density area around hematoma and hematoma were compared between the two groups.Results:The total effective rate of observation group was higher than that of control group(95.00%vs.76.67%,P<0.05).After treatment,NIHSS score,peripheral vascular resistance,MMP-9 and TNF-αin observation group were lower than those in control group(P<0.05),the volume of low-density area around hematoma and hematoma volume in observation group were lower than those in control group(P<0.05),MMSE score and GUSS score in observation group were higher than those in control group(P<0.05).The cerebral blood flow velocity in the observation group was faster than that in the control group(P<0.05),and the cerebral blood flow in the observation group was greater than that in the control group(P<0.05).Conclusion:Qishenhuanwu capsule combined with acupuncture can effectively reduce the neurological damage of patients,improve the cognitive function of patients,improve the swallowing function of patients,alleviate the cerebrovascular function of patients,reduce the level of inflammatory factors,reduce the tissue damage caused by
