摘 要:目的:探究窒息新生儿心肌酶谱水平变化,并分析其与窒息程度、脑损伤的关系。方法:将149例窒息新生儿按照Apgar评分不同分为轻度窒息组(n=103)和重度窒息组(n=46);以同期健康足月新生儿设为对照组(n=60)。比较各组新生儿心肌酶谱水平变化,并分析心肌酶谱水平与窒息程度、脑损伤的相关性。结果:各组新生儿乳酸脱氢酶(LDH)、谷草转氨酶(AST)、羟丁酸脱氢酶(HBDH)、肌酸磷酸激酶(CK)、CK同工酶(CK-MB)水平比较:重度窒息组>轻度窒息组>对照组,差异均具有统计学意义(P<0.05);各组新生儿脑损伤发生率比较:重度窒息组>轻度窒息组>对照组,差异具有统计学意义(P<0.05);Spearman相关检验结果显示,新生儿LDH、AST、HBDH、CK及CK-MB水平与窒息程度及脑损伤均呈正相关(P<0.05)。结论:窒息新生儿心肌酶谱水平升高,心肌酶谱水平变化可以在一定程度上帮助判断新生儿窒息及脑损伤的严重程度,有望成为新生儿预后评估指标。Objective:To explore the changes of myocardial enzymes in asphyxia neonates,and analyze their relationship with asphyxia degree and brain injury.Methods:A total of 149 neonates with asphyxia were divided into mild asphyxia group(n=103)and severe asphyxia group(n=46)according to different Apgar score.A total of 60 healthy full-term neonates were selected as the control group.The changes of myocardial enzymes in groups were compared.The correlation between myocardial enzymes levels and asphyxia degree and brain injury was analyzed.Results:Comparison of the levels of lactate dehydrogenase(LDH),aspartate aminotransferase(AST),hydroxybutyrate dehydrogenase(HBDH),creatine phosphokinase(CK)and CK isoenzyme(CK-MB)in groups:severe asphyxia group>mild asphyxia group>control group,with statistical significance(P<0.05).Comparison of the incidence of neonatal brain injury in groups:severe asphyxia group>mild asphyxia group>control group,with statistically significant differences(P<0.05).Spearman correlation test results showed that the levels of LDH,AST,HBDH,CK and CK-MB in newborns were positively correlated with the asphyxia degree and brain injury(P<0.05).Conclusion:The levels of myocardial enzymes are significantly higher in asphyxia neonates.Observing the changes of myocardial enzymes can help to determine the severity of neonatal asphyxia and brain injury,and it is expected to become a prognostic indicator for newborns.
