点阵模式Q开关1064 nm掺钕钇铝石榴石激光联合透明质酸治疗面部光老化的疗效观察

摘　　要：目的:评价点阵模式Q开关1064 nm掺钕钇铝石榴石(Nd:YAG)激光联合透明质酸治疗面部光老化的临床疗效及安全性。方法:对20例临床诊断为面部光老化的女性受试者,给予点阵模式Q开关1064 nm Nd:YAG激光联合透明质酸治疗,每4周1次,共5次。采用光老化整体评分(GSP)法对受试者治疗前、后面部光老化程度进行评价。采用Visia图像分析系统和Antera 3D皮肤检测法,评估面部皱纹、纹理、色斑及毛孔的变化,采用视觉模拟评分(VAS)法评估术中疼痛程度。分别于第3次和第5次治疗后进行受试者满意度自评。结果:20例受试者均完成治疗及随访,治疗前、治疗3次及治疗5次后GSP评分分别为(2.55±0.83)、(2.15±0.67)及(1.75±0.79)分,差异有统计学意义(F=24.65,P<0.05)。Visia检测显示治疗前、治疗3次及治疗5次后受试者毛孔、皱纹、色素及纹理评分的差异均有统计学意义(P<0.05);Antera 3D图像检测结果示受试者治疗前、治疗3次和治疗5次后各指标评分的差异均有统计学意义(P<0.05)。治疗3次后,受试者疗效自我满意率为50%;治疗5次后为85%。所有患者VAS评分均值为(3.70±1.03)分,属轻度疼痛,均在可耐受范围。结论:点阵模式Q开关1064 nm Nd:YAG激光联合透明质酸治疗面部光老化可有效改善受试者毛孔、色素、皱纹和纹理,且治疗安全性高,舒适度良好。Objective:To evaluate the clinical efficacy and safety of fractional Q-switched 1064 nm Nd:YAG laser combined with hyaluronic acid in the treatment of facial photoaging Methods:Fractional Q-switched 1064 nm Nd:YAG laser combined with hyaluronic acid was used to treat 20 patients with clinically diagnosed facial photoaging once every 4 weeks for a total of 5 times.During each follow-up period,global scores for photoaging(GSP)were used to evaluate the degree of photoaging on the whole face.Visia image analysis system and Antera 3D skin detection were used to evaluate the improvement of facial wrinkles,glossiness,color spots,pores,before and after treatment.The visual analogue scale(VAS)to assess pain induced by treatment.After 3 and 5 treatments,self-assessment on the degree of satisfaction with therapeutic effects were conducted in subjects.Results:All 20 subjects complet-ed the treatment and follow-up.The GSP scores were 2.55±0.83,2.15±0.67 and 1.75±0.79 before,after 3 treatments and after 5 treatments,respectively,with statistically significant dfferences(F-24.65,P<0.05).Visia test showed improvement in pores,wrinkles,pigmentation and texture before treatment,after 3 treatments and after 5 treatments,respectively,with statistically significant differ ences(P<0.05);Antera 3D image test results showed statistically significant differences between subjects'indicators before treatment,after 3 treatments and after 5 treatments(P<0.05).The self-satisfaction rate was 50%after 3 treatments and 85%after 5 treatments.In addition,VAS pain score was 3.70+1.03,indicating mild pain and tolerable range.Conclusion:The fractional 1064nm Q-switched Nd:YAG laser combined with hyaluronic acid is effective in the treatment of facial photoaging,including facial pores,pigments,wrin-kles and textures.The treatment is safe,effective,and comfortable.
