男性脑血流动力学与中医体质的相关性分析

摘　　要：目的:探讨男性脑血流动力学与中医体质的相关性,为制订脑卒中的防控策略提供依据。方法:以1749名男性健康体检人群为研究对象,收集一般资料,进行脑血流动力学检测及中医体质辨识,分析脑血管功能(CVHI)积分与中医体质类型的相关性。结果:男性CVHI积分随着年龄增长有明显的下降趋势,男性CVHI异常率随着年龄增长有明显升高的趋势;男性CVHI积分较低的偏颇体质为痰湿质[(74.20±28.25)分]、血瘀质[(74.88±28.31)分]和阴虚质[(77.65±25.16)分];男性偏颇体质在CVHI积分中最差的分别是痰湿质(V_(mean)左)、痰湿质(V_(mean)右)、血瘀质(V_(min)左)、血瘀质(V_(min)右)、阴虚质(Wv左)、血瘀质(Wv右)、痰湿质(Rv左)、痰湿质(Rv右)。经二分类Logistic回归分析,筛选出气虚质、阴虚质、痰湿质、血瘀质、特禀质为CVHI异常的危险因素,其OR值分别为3.338、3.061、4.517、4.286、3.780。结论:气虚质、阴虚质、痰湿质、血瘀质、特禀质等偏颇体质是男性脑卒中预防的重点人群。在脑卒中防治中,应重视体质辨识,针对脑血管血流动力学指标进行风险评估,早期识别高危个体,通过体质调理达到未病先防。Objective:To explore the correlation between cerebral hemodynamics and traditional Chinese medicine(TCM)constitution in male,and to provide evidence for prevention and control of stroke.Methods:The general information,cerebral hemodynamics and TCM Constitution were collected for the 1749 subjects.The correlation between cerebral vascular hemodynamic indexes(CVHI)and TCM constitution type was analyzed.Results:Significant decreasing trends of CVHI integral score and increasing trends of abnormality rate of CVHI were observed with the increment of age among the subjects.The CVHI integral score of phlegm dampness(74.20±28.25),blood stasis(74.88±28.31)and yin deficiency(77.65±25.16)were lower in all of the biased TCM constitution.The lowest scores of biased TCM constitution in V_(mean)(left),V_(mean)(right),V_(min)(left),V_(min)(right),Wv(left),Wv(right),Rv(left)and Rv(right)were phlegm dampness,phlegm dampness,blood-stasis,blood-stasis,yin deficiency,blood-stasis,phlegm dampness and phlegm dampness respectively.Binary logistic regression analysis determined that qi-deficiency,yin-deficiency,phlegm-dampness,blood-stasis and special-nature were the risk factors of abnormality of CVHI,and that the OR values were 3.338,3.061,4.517,4.286 and 3.780 respectively.Conclusion:Qi-deficiency,yin-deficiency,phlegm-dampness,blood-stasis and special-nature are the key male population in cerebrovascular health management.More attention should be paid on TCM constitution identification in the prevention and treatment of stroke.In addition,it is important to evaluate the risk of stroke with the cerebrovascular hemodynamic indexes,and identify high-risk individuals in the early stage,for prevention before illness according to adjusting constitution.
