痤疮患者外周血IL-17、TNF-α及Th22细胞水平与痤疮严重程度的相关性研究

摘　　要：目的探究痤疮患者外周血白介素-17(IL-17)、肿瘤坏死因子α(TNF-α)及外周血辅助性T细胞22(Th22)细胞水平与痤疮严重程度的相关性。方法选择2020年5月至2022年1月门诊进行治疗的痤疮患者78例作为观察组,同一阶段体检的健康体检者47例作为对照组,比较2组外周血IL-17、TNF-α、IL-22、Th22细胞水平。按Pillsbury法将78例痤疮患者分为轻度(Ⅰ级)组19例,中度(Ⅱ~Ⅲ级)组39例,重度(Ⅳ级)组20例,比较3组血清外周血IL-17、TNF-α、IL-22、Th22细胞水平。并进一步分析外周血IL-17、TNF-α及Th22细胞水平与痤疮严重程度的相关性。结果相较于对照组,观察组IL-17、TNF-α、IL-22及Th22细胞水平均明显上调(P<0.05)。相较于轻度组,中、重度组IL-17、TNF-α、IL-22、Th22细胞水平均明显上调(P<0.05);相较于中度组,重度组IL-17、TNF-α、IL-22、Th22细胞水平均明显上调(P<0.05)。IL-17、TNF-α、Th22预测痤疮严重程度的曲线下面积(AUC)分别为0.736、0.750、0.849,灵敏度分别为94.74%、95.53%、100.00%,特异度分别为51.28%、61.54%、71.79%。IL-17、TNF-α、Th22细胞水平与痤疮患者严重程度均呈正相关(r=0.544,0.443,0.691,P=0.032,0.040,0.021)。结论痤疮患者外周血中IL-17、TNF-α、Th22细胞水平均明显上调,与病情严重程度相关,随病情严重程度增加而上调,在预测痤疮严重程度方面具有重要价值。Objective To explore the correlation between the peripheral blood levels of interleukin-17(IL-17)and tumor necrosis factor-α(TNF-α)and T helper cells type 22(Th22)cell count,with the severity of acne.Methods A total of 78 acne patients treated in the outpatient department of our hospital from May 2020 to January 2022 were selected as the observation group,and 47 healthy subjects who underwent physical examination in our hospital during the same period were selected as the control group.Peripheral blood levels of IL-17 and TNF-αand Th22 cell count were compared between groups.According to the Pillsbury method,78 acne patients were divided into mild group(gradeⅠ,n=19),moderate group(gradeⅡ~Ⅲ,n=39),and severe group(grade-Ⅳ,n=20).Peripheral blood levels of IL-17 and TNF-αand Th22 cell count were further compared in the three subgroups.The correlation between the peripheral blood levels of IL-17 and TNF-αand Th22 cell count with the severity of acne was analyzed.Results Compared with those of the control group,peripheral blood levels of IL-17 and TNF-α,and Th22 cell count in the observation group were significantly elevated(all P<0.05).Compared with those of the mild group,peripheral blood levels of IL-17 and TNF-α,and Th22 cell count in the moderate and severe groups were significantly elevated(all P<0.05),which were significantly higher in the severe group than those of moderate group(all P<0.05).The area under the curve(AUC)of peripheral blood levels of IL-17 and TNF-α,and Th22 cell count for predicting the severity of acne was 0.736,0.750,and 0.849,respectively,with the sensitivity of 94.74%,95.53%,and 100.00%,and the specificity of 51.28%,61.54%,and 71.79%,respectively.Peripheral blood levels of IL-17 and TNF-α,and Th22 cell count were positively correlated with the severity of acne(r=0.544,0.443,and 0.691,respectively;P=0.032,0.040,and 0.021,respectively).Conclusion Peripheral blood levels of IL-17 and TNF-α,and Th22 cell count are significantly elevated in acne patients,which were positively c
