摘　　要：目的:探讨复合手术开通症状性复杂颈动脉闭塞病变的手术技术及近期疗效。方法:回顾性收集2018年4月—2021年5月武汉大学人民医院血管外科收治并成功应用复合手术技术开通症状性复杂颈动脉闭塞患者的临床资料,其中放疗损伤导致闭塞3例,动脉粥样硬化闭塞5例。结果:8例患者复合手术重建全程颈总动脉闭塞均顺利完成,其中1例颈内动脉闭塞开通失败,术后临床症状均得到改善,围术期发生心动过缓、低血压1例,高灌注综合征1例,颈部血肿1例。随访时间为3~39个月,再狭窄1例,为轻度狭窄,未见严重脑卒中,无死亡病例。结论:复合手术是治疗症状性复杂颈动脉闭塞安全有效的方法,近期疗效良好。Objective:To explore the surgical technique and short-term effect of combined surgery for symptomatic complex carotid artery occlusion.Methods:The clinical data of patients with symptomatic complex carotid artery occlusion treated by vascular surgery department of the Renmin Hospital of Wuhan University from April 2018 to May 2021 were collected retrospectively,including 3 cases of occlusion caused by radiation injury and 5 cases of atherosclerotic occlusion.Results:The reconstruction of common carotid artery occlusion was successfully completed in 8 patients,and the opening of internal carotid artery occlusion failed in 1 case.The clinical symptoms of all patients were improved.There were 1 case of bradycardia and hypotension,1 case of hyperperfusion syndrome,and 1 case of cervical hematoma in the perioperative period.The follow-up time was 3-39 months.There was 1 case of restenosis(mild stenosis).No serious stroke and death cases were found.Conclusion:Compound surgery is a safe and effective method for the treatment of symptomatic complex carotid artery occlusion.
