摘　　要：目的:探讨创伤性闭合性跟骨骨折患足抬高高度及结合使用乳酸依沙吖啶液湿敷的消肿效果。方法:选取骨科2018年3月—2021年3月创伤性闭合性跟骨骨折患者160例,按照入院时间顺序随机分为对照组和3个观察组,每组40例。对照组采用布朗氏牵引架抬高患足20 cm和常规消肿方法(冰敷、功能锻炼、20%甘露醇静脉滴注);观察组分为3组,观察1组在对照组基础上结合乳酸依沙吖啶液局部持续湿敷;观察2组将对照组中患足抬高20 cm调整为患足抬高30~40 cm;观察3组在观察2组的基础上,结合乳酸依沙吖啶液局部持续湿敷。比较4组患者术前消肿时间。结果:4组患者入院时肿胀评分比较差异无统计学意义(P>0.05),而在消肿天数、住院时间方面比较差异有统计学意义(P<0.001)。3个观察组患者的消肿效果均优于对照组,观察2组优于对照组和观察1组,观察3组优于观察2组。结论:闭合性跟骨骨折后患足抬高30~40 cm并结合使用乳酸依沙吖啶液湿敷可缩短患足消肿时间。Objective:To explore the effect of elevation of the affected foot in traumatic closed heel fractures and the combination of wet compresses with estradiol lactate solution on the reduction of swelling.Methods:All 160 patients with traumatic closed heel fractures from March in 2018 to March in 2021 were selected and randomly divided into four groups of 40 cases each according to the admission time order.The patients in the control group were treated with Brown's traction frame raising the affected foot 20 cm and conventional methods(ice compress,functional exercise,20%mannitol intravenous drip).The trial group was divided into 3 groups,observation group 1 combined with ethacridine lactate solution on the basis of the control group,observation group 2 raised the affected foot from 20 cm to 30~40 cm in the control group;observation group 3 on the basis of observation group 2 combined with ethacridine lactate solution local continuous wet compress.Compare the preoperative swelling reduction time of 4 groups of patients.Results:There was no significant difference in swelling score at admission between the four groups(P>0.05),but there was significant difference in the days of detumescence and length of stay(P<0.001).The effect of the three groups was better than that of the control group.The effect of the two groups was better than that of the control group and the first group.The effect of the three groups was better than that of the two groups.The observation group 2 was superior to the control group and the observation group 1,and the observation group 3 was superior to the observation group 2.Conclusion:Elevation of the affected foot at a height of 30~40 cm after a closed heel fracture combined with the use of a wet compress of estaacridine lactate can shorten the swelling time of the affected foot.
