尖锐湿疣病灶内注射免疫治疗的研究进展

摘　　要：尖锐湿疣是由人乳头状瘤病毒感染引起的以生殖器、肛门及周围皮肤黏膜疣状增生性病变为主要临床表现的性传播疾病。目前的治疗方案包括药物治疗、物理治疗、手术治疗、光动力疗法等。每种方法都有其优点和局限性,尚无一种治疗方法适用于所有患者,并且存在较高的复发率。激光和手术等破坏性的治疗方法容易产生瘢痕,并且不适用于大面积病变的患者。病灶内注射免疫治疗可上调患者的机体免疫治疗尖锐湿疣并降低复发率。2021年版《中国尖锐湿疣临床诊疗指南》中病灶内干扰素注射治疗方法被评为强烈推荐(证据等级为A)。除了干扰素,一些疫苗或者免疫原性物质也被用于病灶内注射免疫治疗。不同研究使用的治疗方案和治疗效果也有所不同。本文就目前被用于尖锐湿疣病灶内注射免疫治疗的不同方案进行综述。Genital warts are sexually transmitted diseases caused by human papillomavirus infection with verrucous hyperplastic lesions of the genitals,anus and surrounding mucous skin as the main clinical manifestations.The treatment period of genital warts is long and easy to recur,and the traditional treatment options include drug treatment,physical therapy,surgical treatment,photodynamic therapy,etc.Each treatment has advantages and limitations,and no treatment is suitable for all patients.In addition to destructive treatments such as laser and surgery,it is also an effective treatment method to clear the virus by enhancing host autoimmunity.And due to improve the immunity of patients,the recurrence rate of condyloma acuminatum is also reduced.Topical imiquimod,oral cimetidine,and intralesional injection therapy can modulate systemic or local immunity and are used to treat condyloma acuminatum.Compared with topical administration,intralesional injection can improve the drug concentration and action time in the lesion,thereby enhancing the therapeutic effect.In the 2021 edition of the Chinese Guidelines for the Clinical Diagnosis and Treatment of Genital Warts,intralesional interferon injection was rated as highly recommended(level of evidence A).In addition to interferon,some vaccines or immunogenic substances are also used for the treatment of condyloma acuminatum by intralesional injection.The regimens used and the effects of treatment vary among different studies.This article reviews the different regimens currently used for intralesional immunotherapy of genital warts.
