麻杏石甘汤加减治疗肺热型社区获得性肺炎临床观察

摘　　要：目的 探究麻杏石甘汤加减治疗肺热型社区获得性肺炎的临床效果及对血清降钙素原(PCT)、C反应蛋白(CRP)的影响。方法 选择本溪市中医院2020年7月—2021年7月收治的87例肺热型社区获得性肺炎患者,以随机数字表法分为对照组(43例)和治疗组(44例),分别实施常规治疗、常规治疗联合麻杏石甘汤治疗,比较2组治疗效果、症状消失时间、PCT水平、CRP水平及不良反应发生率。结果 治疗组总有效率为97.73%(43/44),高于对照组的83.72%(36/43)(P <0.05);治疗组体温稳定时间、双肺啰音消失时间、咳嗽消失时间、气急消失时间均较对照组短(P <0.05);治疗后治疗组PCT、CRP水平均较对照组低(P <0.05);治疗组不良反应发生率为6.82%(3/44),与对照组的9.30%(4/43)相近(P>0.05)。结论 对肺热型社区获得性肺炎患者应用麻杏石甘汤加减,可提升治疗效果、缩短病程、降低患者炎症反应水平,且安全性理想。Objective To explore the clinical effect of Maxing Shigan decoction in the treatment of community-acquired pneumonia of pulmonary heat and its influence on serum procalcitonin(PCT)and C-reactive protein(CRP).Methods 87 patients with community-acquired pneumonia with pulmonary heat in Benxi Hospital of Traditional Chinese Medicine from July 2020 to July 2021 were randomly divided into control group(43 cases)and treatment group(44 cases).The control group was treated with routine treatment,and the treatment group was treated with routine treatment combined with Maxing Shigan decoction.The therapeutic effect,symptom disappearance time,PCT,CRP levels and incidence of adverse reactions were compared between the two groups.Results The total effective rate of the treatment group was 97.73%(43/44),which was higher than that of the control group(83.72%,36/43)(P<0.05).The temperature stabilization time,the disappearance time of rales,cough and shortness of breath in the treatment group were shorter than those in the control group(P<0.05).After treatment,PCT and CRP levels in the treatment group were lower than those in the control group(P<0.05).The incidence of adverse reactions in the treatment group was 6.82%(3/44),which was similar to 9.30%(4/43)in the control group(P>0.05).Conclusion The application of modified Maxing Shigan decoction to patients with community-acquired pneumonia with pulmonary heat can improve the therapeutic effect,shorten the course of the disease,reduce the level of inflammatory response of patients,and the safety is ideal.
