温肾健脾 化瘀行水法治疗脾肾阳虚夹瘀型原发性肾病综合征临床观察

摘　　要：目的 探讨温肾健脾、化瘀行水法治疗脾肾阳虚夹瘀型原发性肾病综合征的临床效果。方法 将71例脾肾阳虚夹瘀型原发性肾病综合征患者随机分为治疗组36例,对照组35例。对照组给予西医基础治疗,治疗组在对照组基础上,加用温肾健脾、化瘀行水方,疗程6个月。观察并比较2组治疗前后临床效果、血浆白蛋白、血脂、24 h尿蛋白定量的变化情况。结果 治疗组总有效率为86.11%(31/36),明显高于对照组的62.86%(22/35)(P <0.01)。治疗组各项实验室指标同本组治疗前及对照组治疗后比较,差异有统计学意义(P <0.05或P <0.01)。治疗组中医证候总有效率为91.67%(33/36),对照组为65.71%(23/35),2组治疗后比较,差异有统计学意义(P <0.01)。结论 温肾健脾、化瘀行水法治疗脾肾阳虚夹瘀型原发性肾病综合征患者在临床中取得了满意的效果。Objective To explore the clinical effect of warming kidney,strengthening spleen and removing blood stasis method in the treatment of primary nephrotic syndrome of spleen-kidney yang deficiency with blood stasis type.Methods 71 patients with primary nephrotic syndrome of spleen-kidney yang deficiency with blood stasis type were randomly divided into treatment group(36 cases)and control group(35 cases).The control group was treated with basic Western medicine.On the basis of the control group,the treatment group was treated with warming kidney,strengthening spleen and removing blood stasis recipe for 6 months.The changes of clinical effect,plasma albumin,blood lipid and 24h urinary protein were observed and compared between the two groups before and after treatment.Results The total effective rate of the treatment group was 86.11%(31/36),which was significantly higher than that of the control group(62.86%,22/35)(P<0.01).The laboratory indexes of the treatment group were significantly different from those of the control group before treatment and after treatment(P<0.05 or P<0.01).The total effective rate of traditional Chinese medicine syndromes in the treatment group and the control group was 91.67%(33/36)and 65.71%(23/35),respectively,and the difference was statistically significant(P<0.01).Conclusion Warming the kidney and strengthening the spleen,removing blood stasis method has achieved satisfactory clinical effect in the treatment of primary nephrotic syndrome of spleen-kidney yang deficiency with blood stasis type.
