中药保留灌肠配合护理干预治疗慢性乙型重型肝炎临床观察

摘　　要：目的 探讨中药保留灌肠配合护理干预治疗慢性乙型重型肝炎的效果。方法 将2020年2月—2021年1月厦门市中医院收治的慢性乙型重型肝炎合计80例患者随机分为对照组和研究组各40例,分别予以常规对症治疗及护理和中药保留灌肠配合护理干预。比较2组治疗前后乙肝病毒学指标和乙肝病毒标志物指标、肝功能指标及生活质量变化。结果 2组治疗2周后乙型肝炎病毒的脱氧核糖核酸(HBV-DNA)、乙型肝炎e抗原(HBe Ag)恢复至正常水平的达标率对比差异无统计学意义(P>0.05);研究组治疗4周后2项指标恢复至正常水平的达标率高于对照组(P <0.05)。治疗前,2组肝功能指标及生活质量对比差异无统计学意义(P>0.05);治疗后,研究组总胆红素(TBil)、天冬氨酸转氨酶(AST)、总胆汁酸(TBA)、丙氨酸转氨酶(ALT)均低于对照组,且研究组治疗后生活质量评分高于对照组(P <0.05)。结论 中药保留灌肠配合护理干预可提高慢性乙型重型肝炎的治疗效果,改善肝功能水平,进而提高其生活质量。Objective To discuss the clinical effect of Chinese herb retention enema therapy and nursing intervention in treating the chronic severe hepatitis B.MethodsA total of 80 patients with chronic severe hepatitis B admitted to Xiamen Hospital of Traditional Chinese Medicine from February 2020 to January 2021 were randomly divided into a control group of 40 cases and a study group of 40 patients.The control group received conventional symptomatic treatment,and the study group was given Chinese herb retention enema therapy and nursing intervention.The changes of hepatitis B virology index,hepatitis B virus marker index,liver function index and quality of life before and after treatment were compared between the two groups.Results After 2 weeks of treatment,there was no significant difference in the compliance rate of hepatitis B virus deoxyribonucleic acid(HBV-DNA)and hepatitis B e antigen(HBeAg)to the normal level(P>0.05).After 4 weeks of treatment,the compliance rate of 2 indexes in the study group was higher than that in the control group(P<0.05).Before treatment,there was no significant difference in liver function index and quality of life between 2 groups(P>0.05).After treatment,the total bilirubin(TBil),aspartate aminotransferase(AST),total bile acid(TBA)and alanine aminotransferase(ALT)of the study group were lower than those of the control group,and the quality of life score of the study group after treatment was higher than that of the control group(P<0.05).Conclusion Chinese herb retention enema therapy and nursing interventions can improve the curative effects,liver function and life quality of chronic severe hepatitis B patients.
