从正邪关系视角重新认识玫瑰痤疮

摘　　要：此文通过整理中西医对玫瑰痤疮的认识,发现古今中医对玫瑰痤疮的病因病机主要是从热邪这一邪实立论,没有从气血阴阳等正气与邪气动态关系角度进行深入探讨;西医认为玫瑰痤疮有多种致病因素,包括易感基因等内因和微生物等外因,二者相互作用导致玫瑰痤疮的发生。这与中医“正邪相争”的认识在本质上一致,进一步进行了深入分析,包括发生玫瑰痤疮者多为阴虚体质。在此基础上,提出滋阴凉血、解毒化斑的治疗原则,并以自拟滋阴凉血解毒汤为例进行说明,从正邪关系的角度重新认识了玫瑰痤疮的发病机制和治疗。Through the collection and classification of the understanding of traditional Chinese medicine and Western medicine toward rosacea,we re-understand the mechanism and therapy of rosacea from the perspective of relationship of pathogenic factors and healthy qi.We firstly state the different understanding of rosacea between traditional Chinese medicine and Western medicine,figure out that traditional Chinese medicine considers pathogenic heat to be the mechanism of rosacea while do not discuss the dynamic relationship between qi,blood,yin,yang and other pathogenic qi and evil qi.Secondly,we find that Western medicine considers that there are many determinants of the pathogenic of rosacea,including internal factors such as susceptibility genes and external factors such as microorganisms,which interact with each other to cause rosacea.The essence of Western medicine’s consideration is consistent with that of traditional Chinese medicine.This research has a further analysis and figures out that the occurrence of rosacea is mostly on yin deficiency constitution.Based on this conclusion,we propose therapeutic principles of nourishing yin and cooling blood,and relieving poison and spots.The self-made Ziyin Liangxue Jiedu decoction were extracted as an example to illustrate.
