摘  要:目的:分析MRI短时反转恢复(STIR)序列诊断急性隐匿性椎体损伤的临床价值。方法:择取2019年1月至2020年12月本院急性脊柱创伤病人89例,进行自旋回波序列T1WI、快速自旋回波T2WI序列、STIR序列的MRI检查,分析病人隐匿性椎体损伤状况。结果:89例患者中,T2WI序列检出210节椎体异常高信号(正常形态124节,形态异常86节),STIR序列检出250节异常高信号(正常形态164节,形态异常86节),STIR序列较T2WI序列多检出40节(P<0.05)。89例病人中出现继发性不相邻损伤椎体节段146节,损伤部位以胸椎损伤最为常见(占38.36%),病因以骨挫伤为最常见(占36.99%)。结论:相较于T2WI,MRI STIR序列更为全面、准确地评估急性脊柱创伤后隐性椎体受损,对于无形态改变的椎体损伤更为敏感,可早期全面地评估继发性、多水平、不相邻椎体损伤,对指导治疗方案及评估预后方面具有重要意义。Objective:To evaluate the value of MRI short-time reversal recovery(STIR)sequences in the.evaluation of hidden vertebral body injury after acute spinal trauma.Methods:From January 2019 to December 2020,89 cases of patients with acute spinal trauma in our hospital were selected.All patients underwent MRI examination of spin echo sequence T1 WI,fast spin echo sequence T2WI and stir sequence.The status of occult vertebral body injury was analyzed.Results:In 89 patients,210 cases of vertebrae with abnormally high signal intensity were dected on T2WI(124 normal and 86 abnormal),and 250 cases of vertebrae with abnormally high signal intensity were detected on STIR sequence(164 normal and 86 abnormal).STIR sequence detected 40 more nodes than T2WI sequence(P<0.05),Among the 89 patients,146 vertebral body segments had secondary non-adjacent injury,the most common site of injury was thoracic injury(38.36%),and the most common cause was bone contusion(36.99%).Conclusion:compared with T2WI,stir sequence MRI is more comprehensive and accurate in evaluating the occult vertebral body injury after acute spinal trauma,and is more sensitive to the vertebral body injury without morphological changes.It can comprehensively evaluate the secondary multi-level non adjacent vertebral body injury early,which is of great significance in guide the treatment plan and evaluate the prognosis.
