摘  要:目的:探讨婴幼儿优化3D三维快速自转回波成像技术(3D-Sampling Perfection with Application-Optimized Contrasts by using Different Flip Angle Evolutions, 3D-SPACE)序列磁共振尿路成像技术(magnetic resonance urography, MRU)与常规3D-SPACE MRU在图像质量和采集时间方面的差异。方法:分析2016年6月至2020年4月0~5岁44例婴幼儿肾盂积水在我院行优化3D-SPACE MRU和常规3D-SPACE MRU序列检查,扫描技师记录采集时间,并客观测量、计算肾盂及肝脏的噪声、信噪比(SNR)及对比噪声比(CNR)。两位放射科医生分析整体图像质量、运动伪影,并进行伪影评分及清晰度评分。采用统计学方法比较两组的采集时间、噪声、SNR、CNR、伪影评分及清晰度评分。结果:所有MRU检查均顺利完成。优化3D-SPACE MRU的采集时间为165±55.90s,而常规3D-SPACE MRU采集时间为359±66.64s,常规MRU与优化MRU噪声、SNR和CNR分别为(1.94±0.86、0.93±0.24;206.20±90.20、366.50±93.30;162.50±98.20、358.80±93.50),这四项均有统计学差异(P<0.05),与常规MRU相比,优化MRU扫描时间更短,噪声更小,SNR及CNR更高。两组伪影评分及清晰度评分无统计学差异(P值分别为0.80及0.36)。结论:优化3D-MRU相对常规3D-MRU更有优势,减少了采集时间并提高了婴幼儿肾盂积水检查的图像质量。Objective:To explore the difference of image quality and acquisition time between optimized 3D-SPACE(3D-Sampling Perfection with Application-Optimized Contrasts by using Different Flip Angle Evolutions)sequence MRU(magnetic resonance urography,MRU)and conventional 3D-SPACE sequence MRU for infants.Methods:44 cases of infants with hydronephrosis aged Oto 5 years old from June 2016 to April 2020 in our hospital wereanalyzed.Optimized 3DSPACE MRU and routine 3D-SPACE MRU sequence examinationswere performedinall the infants.Technicians recorded the acquisitiontime,and objectively measured and calculated the noise,signal to noise ratio(SNR)and contrast to noise ratio(CNR)of renal pelvis and liver.The image quality,motion artifacts,the artifact scores and image definition scores were analyzed by two radiologists.Statistical methods were used to compare the acquisition time,noise,SNR,CNR,the artifact scores and image definition scores between the two groups.Results:All MRU examinations were completed successfully.The acquisition time of optimized 3D-SPACE MRU was 165±55.90s,while that of conventional 3D-SPACE MRU was 359±66.64s.The noise,SNR and CNR of conventional MRU and optimized MRU were 1.94±0.86 and 0.93±0.24,206.20±90.20,366.50±93.30;162.50±98.20,358.80±93.50,respectively.There were statistical differences in these four items(P<0.05).Compared with conventional MRU,optimized MRU had shorter acquisition time,lower noise,and higher SNR and CNR.There was no statistical difference in the artifactscoresandimage definition scores between the two groups(P value was 0.80 and 0.36,respectively).Conclusion:Compared with conventional 3D-MRU,optimized 3D-MRU has more advantages.Optimized 3D-MRUcan reduce the acquisition time and improve the image quality of hydronephrosisin infants.
