分段缝扎悬吊结合外剥内扎术和痔科生肌散治疗环状混合痔临床研究

摘　　要：目的:探讨分段缝扎悬吊结合传统外剥内扎术对肛门功能的影响以及痔科生肌散对术后创面愈合的影响。方法:将90例环状混合痔患者随机分为观察A组、观察B组和对照组,每组30例。观察组A组患者采用分段缝扎悬吊结合外剥内扎术治疗,术后外用痔科生肌散;观察B组患者采用分段缝扎悬吊结合外剥内扎术治疗,术后常规换药;对照组患者采用外剥内扎术治疗,术后常规换药。术后第3天进行疼痛、水肿评分,术后第5天检测血清中的白细胞数、C-反应白蛋(CRP)、血清降钙素原(PCT),统计创面愈合时间及肛门狭窄率。结果:观察A组和观察B组患者疼痛、水肿评分以及肛门狭窄率均明显低于对照组(P<0.05),观察A组创面愈合时间明显短于观察B组和对照组(P<0.05),观察A组CRP、PTC明显低于观察B组和对照组(P<0.05)。结论:分段缝扎悬吊术能够有效减轻术后疼痛和水肿,更好地保护肛管皮肤,减少肛门狭窄;术后结合痔科生肌散能够减轻术后创面炎症反应,促进创面愈合。Objective To investigate the influence of segmental suture-ligature-suspension combined with external dissection&internal ligature on anal function,as well as the influence of Zhike Shengji San(San,a TCM formula)on postoperative wound-surface healing.Methods Randomly divided the 90circumferential mixed hemorrhoids(CMH)patients treated in author's hospital into three groups:observation group A(30cases,received segmental suture-ligature-suspension combined with external dissection&internal ligature,afterwards San for externally using),observation group B(30cases,surgery same to group A,only after surgery,routine dressing exchange),and control group(30cases,only external dissection&internal ligature,routine dressing exchange after surgery);on the 3rd day after surgery made the score ratings on pain,and edema;on the 5th day after surgery detected patients'serum WBC,CRP,and PCT,also,statistically recorded wound-surface healing time and the ratio of anal stenosis.Results In the ratings on pain and edema and the ratio of anal stenosis observation group A and B were all significantly lower than control group(P<0.05);in wound-surface healing time group A was significantly shorter than rest two groups(P<0.05),also,in the levels of CRP and PCT,was significantly lower than rest two groups(P<0.05).Conclusion Segmental suture-ligature-suspension can effectively reduce postoperative pain and edema intensity,better protect anal canal's skin,decrease anal stenosis extent;after surgery using San for dressing exchange can relieve the postoperative inflammatory response of wound-surface,further,promote its healing.
