基于“肾与膀胱相表里”论治妇科疑难病

摘　　要：根据中医基础理论,肾主封藏,其核心功能为“收涩”,膀胱属腑,以通降为顺,其核心功能为“通利”。根据脏腑的表里理论,肾的“收涩”与膀胱的“通利”相辅相成。通过对“五子衍宗丸”等方剂利用利尿药来达到“固肾”目的的解析,作者得出结论:在一定程度上,通过“利尿”可以增强肾的“固涩”功能。反之,通过“固肾”同样也可促进膀胱的“排尿”功能。在肾虚型崩漏及漏尿等妇科疑难杂症的治疗中,除了在辨证的基础上“补肾固肾”外,还应酌加“利尿”中药,以增强肾的收涩功能。在治疗肾虚型女性排尿困难的“劳淋”时,除了在辨证的基础上“补肾”及“利尿”外,还应酌加“固肾”的中药,以增强膀胱的排尿功能。According to the basic theory of traditional Chinese medicine,the kidney governs storage,and its core function is “retracting astringent”,while the bladder belongs to the internal organs,and its core function is “tongli”.According to the internal and external theory of viscera,the “squeezing” of the kidney and the “tongli” of the bladder complement each other.Through the analysis of “Wuzi Yanzong Pills” and other prescriptions that use diuretics to achieve the purpose of “solidifying the kidney”,the author draws a conclusion:to a certain extent,the function of “consolidating astringency” of the kidney can be enhanced by “diuretic”.Conversely,the “urination” function of the bladder can also be promoted by “strengthening the kidney”.In the treatment of intractable gynecological diseases such as metrorrhagia and urinary leakage due to kidney deficiency,in addition to “reinforcing the kidney and strengthening the kidney” on the basis of syndrome differentiation,traditional Chinese medicine “diuretic” should also be added to enhance the astringent function of the kidney.In the treatment of dysuria in women with kidney deficiency,in addition to “tonifying the kidney” and “diuresis” on the basis of syndrome differentiation,traditional Chinese medicine “strengthening the kidney” should also be added to enhance the urinary function of the bladder.
