安神定志丸治疗高血压伴围手术期焦虑的临床研究

摘　　要：目的:观察安神定志丸治疗高血压伴围手术期焦虑患者的临床疗效。方法:选取2021年6月—2022年5月在广州中医药大学附属北碚中医院住院且符合研究标准的患者60例,随机分为观察组与对照组,每组30例,两组均给予控制血压及围手术期的西医基础治疗,观察组联合安神定志丸治疗,对照组联合地西泮片治疗,比较治疗前后两组的焦虑自评量表(SAS)及视觉模拟焦虑评分法(VAS)评分、血压、心率、肾上腺素(E)、去甲肾上腺素(NE)、血管紧张素Ⅱ(AngⅡ)的监测。结果:通过治疗后第5、7天,观察组的SAS及VAS焦虑评分均低于对照组,观察组的收缩压(SBP)控制优于对照组(P<0.05);治疗后第7天,观察组的心率控制优于对照组(P<0.05);在检验指标上,两组间的E、NE值比较差异无统计学意义(P>0.05),观察组的AngⅡ值(第5天)明显低于对照组(P<0.05),观察组内比较,治疗后的E值(第5天)、NE值(第3、5天)、AngⅡ值(第3、5天)显著降低(P<0.05),而对照组在治疗前后的E值、NE值、AngⅡ值差异无统计学意义(P>0.05)。结论:安神定志丸具有降压、控制焦虑的作用,能够提高降压疗效,特别能够降低收缩压,改善高血压患者在围手术期的焦虑程度,可以更好地保障患者围手术期的相关治疗。Objective:To observe the clinical efficacy of Anshen Dingzhi Pill in treatment of hypertension with perioperative anxiety.Methods:60 patients,who met the research criteria,were randomly divided into the observation group and the control group.Both groups were given basic Western medicine treatment for controlling blood pressure and for perioperative period;on which basis,the observation group was treated with Anshen Dingzhi Pill,whereas the control group was treated with Diazepam Tablet.The scores of Self-rating Anxiety Scale(SAS),and Visual Analogue Anxiety Scale(VAS),blood pressure,heart rate,epinephrine(E),norepinephrine(NE)and angiotensinⅡ(AngⅡ)were compared between before and after the treatment in the two groups.Results:On 5th and 7th days of treatment,the scores of SAS and VAS were lower in the observation group than those in the control group,and the control of systolic blood pressure(SBP)was better in the observation group than that in the control group(P<0.05).On 7th day of treatment,the heart rate control of the observation group was better than that of the control group(P<0.05).In terms of test indexes,there were no statistical differences in E and NE between the two groups(P>0.05).The value of AngⅡon 5th day in the observation group was lower than that in the control group(P<0.05).The values of E(5th day),NE(3rd day and 5th day)and AngⅡ(3rd day and 5th day)were significantly lower after the treatment than those before the treatment in the observation group(P<0.05);there were no statistical differences in the values of E,NE and AngⅡbetween before and after the treatment in the control group.Conclusion:Anshen Dingzhi Pill has the effect of reducing blood pressure and controlling anxiety.It can improve the effect of reducing blood pressure,especially reduce SBP and improve the anxiety level of patients with hypertension during perioperative period,which can better ensure the relevant treatment of patients during perioperative period.
