选择性功能动作评估指导下“仰头推正”法治疗颈性眩晕的临床研究

摘　　要：目的:研究选择性功能动作评估(SFMA)指导下“仰头推正”法治疗颈性眩晕的临床疗效。方法:选择符合纳入标准的80例颈性眩晕患者,随机分为两组(各40例)。两组均采用基础治疗,对照组结合常规“仰头推正”法治疗,治疗组结合以选择性功能动作评估为指导的“仰头推正”法治疗,两组患者治疗1次/d,5 d为一个疗程。治疗前后分别对患者进行视觉模拟量表(VAS)评分、颈性眩晕症状与功能评估量表(ESCV)评分,比较观察两组患者的临床疗效、眩晕指数及生活质量方面的改善情况。结果:两组治疗后VAS评分及ESCV评分均优于治疗前,差异有统计学意义(P<0.05),且治疗组效果优于对照组,差异有统计学意义(P<0.05);治疗组有效率及复发率优于对照组,差异有统计学意义(P<0.05)。结论:选择性功能动作评估能有效指导“仰头推正”手法改善颈性眩晕的症状,有一定的临床价值;“仰头推正”手法操作相对简单易行,无副作用,值得临床推广应用。Objective:To study the clinical efficacy of the manipulation of head-up and push-straight with the instruction of selective functional movement assessment(SFMA)on cervical vertigo.Methods:80 patients with cervical vertigo were collected as study subjects according to inclusion and exclusion criteria.They were randomly divided into two groups,each group had 40 cases.All the patients were treated with the basic treatment.The patients of the control group were treated with the manipulation of head-up and push-straight.The patients of treatment group were treated with the manipulation of head-up and push-straight after the instruction of SFMA.Two groups received treatment once a day for 5 d as a course of treatment.The changes of the scores of visual analogue scale(VAS)and evaluation of symptoms and function of cervical vertigo(ESCV)after and before treatment were observed and analyzed in the two groups to compare and observe the improvement of the two groups of clinical efficacy,vertigo index and quality of life.Results:The scores of VAS and ESCV were better after treatment than before the treatment in the two groups,and the difference had statistical meaning(P<0.05).And the treatment group was better than the treatment(P<0.05).The effective rate and the recurrence rate in the treatment group was better than the control group,and the difference had statistical meaning(P<0.05).Conclusion:SFMA can effectively instruct the manipulation of head-up and push-straight to improve the symptoms of cervical vertigo.It has certain clinical value for cervical vertigo.The manipulation of head-up and push-straight is worthy of clinical application for its simple to operate without side efficacy.
